I appreciate how you emphasize finding a diet "that works for you."
I'm currently moving away from my SAD diet because I've long felt terrible. It's a big process to learn everything you need to know, and become accustomed to the new info; and even more difficult and time-consuming, is changing lifelong habits of eating, and keeping to it on a consistent basis. I just politely refused pizza and birthday cake on Friday. It's tough.
I started out on dirty keto, and I'm now moving on to clean keto, and I will further develop and refine my dietary habits as I learn more and adapt my lifestyle to accommodate my new healthy choices.
I say this to reinforce my appreciation for your sentiment about finding the diet that works for you.

JUst because the paper you referenced shows that lowering total LD reduced risk of heart diseases does not disprove the argument that its particularly the small LDL particles that cause heart diseases. Since lowering total LDL lowers concentration of ALL particle sizes

I did my due diligence and followed the guidelines: I took Zetia and reduced saturated intake. It took my total cholesterol from 435 to 215. OxLDL, LDL, ApoB, etc. all lower. But, it completely killed my libido, ruined my sleep, and when I had my doctor test my testosterone, it was low. The reason I eat a diet of beef, eggs, fruit, and a little dairy is because the grains, beans, and chicken that I was eating for their supposed health benefits were tearing up my gut and disrupting my metabolism. At the height of my abdominal pain, switching from coconut oil to olive oil made it worse! Not only did this diet restore my health, it leaned me out for the very first time in my life. I'm lighter, leaner, and stronger than when I graduated high school. Fat adapting has stabilized my moods. At 60, CAC=0 and a CIMT only showed some patches on the carotid bulbs but no blockages. Maybe I'm just a weird outlier, but from my perspective the medical industrial complex is trying to dumb everything down to blanket, one-size-fits-all recommendations that are disastrous for a lot of people. Needless to say, I no longer take dietary advice from clowns practicing iatrogenic medicine.

I have IBS and I find that higher levels of fresh vegetables (green leafy ones in particular) massively spike my gut inflammation, but not every time. I suspect my gut biome is very sensitive to contaminated uncooked foods, which seems to happen quite frequently when I eat out. I find that if I'm making it myself and I am meticulously cleaning my greens first, I have fewer problems. But life is busy and I just don't have time for that most days. And because inflammation and gut health is so critical to overall health, I find it much easier to lean into a more Keto'y diet where I cook most of what I eat. I use a lot of butter/ghee and a lot of olive oil (and yes I heat it up), when I make those kinds of foods I feel the best. What I've changed lately is I've gone down the anti-seed oil path. This highly processed oil is awful for our bodies and from what I understand contributes to making LDL more harmful. I've been avoiding seed oils as much as practical for about 2 months now and I feel better than I have in a long time.

As a side note, I've found I can no longer tolerate beef. That came out of the blue, and it's been really frustrating. And no I don't live in an area where there are ticks, so it's not from a Lone Star tick bite, just totally random as far as I can tell.

Personally I believe that a " Near Vegetarian, - Flexitarian Diet, " is best. Something resembling a Lacto-ovo - vegetarian or Mediterranean Diet, for me, makes me feel the best, look the best, and gives me optimal health outcomes. I've done both strict Vegan and Low Carb in the past....and they worked too, but I felt some negative side effects from both, and I was not able to maintain either for more than a year. Thanks, Doc.

I would really like to see a live debate between two qualified people each taking one side of the argument that sat fat is bad/good, and also statins are harmless/not-harmless. This video makes it sounds like both these issues are already concluded and the non-mainstream people are on the fringe. If that's true, the debate will prove it easily, no reason to fear. Right now each side presents their case smugly as though there is no argument and that helps no one. Lets see a debate.

Gabrielle Lyon promotes a muscle centric diet. She says muscle is the biggest predictor of longevity. I agree. We all need to preserve muscle for strength as we age. Sarcopenia is a real risk - most of us do not eat enough protein.

I think it would be great if food labels had a protein quality score based on the levels of each amino acids. Not all proteins are equal

I ate a doctor and dietitian-recommended ultra-low-fat diet for twenty years. The results were: I gained 100lbs, I got metabolic syndrome, I got 2 BP meds, I got fatty liver, I got terrible acne, I got bad knees, I got premature alopecia, I got inhalers, I got statins, I got PAD, I got sicker and sicker. I was lied to. I went KETO! I lost 70lbs in one year, got off most meds, and no longer had metabolic syndrome. Beware of Tufts and Harvard and other money-motivated liars.

Just a note about the use of the formula for protein consumption. My understanding is that the amount of protein consumed as a function of "mass" is based on lean muscle mass, not total weight. Otherwise, someone with a high-fat percentage would be expected to consume far more than someone with a low-fat percentage. So a person weighing 80kg – with, for instance, 50kg lean muscle mass – should seek to consume approximately 1.6x50=100g of protein per day. But a 100kg person with a high BMI (say, body composition of 35% fat) would only need to consume ~105g per day. In reality, this multiplier appears to be pretty uncertain, with numbers from 1-2 being bandied about. So 1.6 is a reasonable average.

I know you said it’s a modified version, but I still think its worth pointing out that avocados and blueberries are not part of the Mediterranean diet. There are plenty of cheaper and more sustainable alternatives that are though, like grapes, pomegranate, almonds and olive oil. I think the Anglo speaking world needs more exposure to what the Mediterranean diet actually consists of.

1. Get your recommended level of protien as many people are under-protien'ed
2. Lower LDP cholesterol and be aware of your cholesterol level/intake (reduce intake of saturated fats)
3. Increase fibre, decrease sugar/processed food intake
4. Reduce salt intake

I developed heart disease and arthritis while on keto/carnivore diet. It was mainly to gain muscle. I was never over weight to begin with. I’m now on vegan diet with lots of vegetable protein that enables me to gain more muscle. My health is on the mend and my blood panel is looking really great. Thanks for the great summary Dr. Stanfield.
