For any new physios, remember: It all begins with being able to properly sub-classify a patient’s pain in order to then prescribe the right type and dose of movement for the patient. Listen to your patient’s needs and concerns, communicate effectively and give them what they need based on your findings and their unique circumstances. Do this and you’ll be well on your way to a successful career.

What is your opinion on dr. Stuart mcgill? I've started reading some of his books and he seems to be a big advocate of isometric exercises that improve spinal stability to decrease low back pain.

I feel like it’s very hard to do meta analysis across studies for complicated problems like lower back pain as there are many different causes. If you look at 1000 cases, of course it seems like the solution is general not specific. It’s like trying to generalise depression treatment. Anything from exercise, sleep schedule, sun exposure to drugs, childhood trauma resolution can all help. Like you said, it’s case by case. I have had lower back pain for more than a decade and walking for sure didn’t solve it lol. Spent years visiting different “professionals’, from sports specific doctors, pain specialist doctors, physiotherapists, chiropractor, Chinese doctors, stretch specialist and personal trainers to specialists with combo of above and other disciplines. Some of them helped a little but most were useless. In the end the cause was basically a combo of factors too complicated for them to diagnose and let alone treat. I had to study and solve it myself.

For low back pain I focus on postural exercise, core strengthening, and hip/glute strengthening

For me, the best exercise for reducing low back pain is the front plank, contracting the abs as hard as possible for 10-15 sec, then taking a deep breath in, while still maintaining tension and repeating the contraction over and over for a total of 2 min. When I do this 2-3 times per day my low back pain is significantly less.
