If you dissect the scene, as Patrick does to women, he's asking her plenty of questions to find the bad in her and convince himself that she's a horrible person and disgusts him. Her outright caring, charm, and humble honesty shine through but Patrick still feels the need and itch to kill that bubbles to the surface but ultimately decides she doesn't deserve what he'd do to her.

He never kills Jean because she is the only decent person he has met & she actually listens to him. In the book, the only fantasy he has that is not morbid, is buying Jean a bunch of balloons while they run through Central Park.

For anyone confused why he didn’t kill her, it’s not because he likes her or anything, but it’s the fact that he couldn’t find any of the flaws in her that he basically saw every other person. Right before he kills her he hesitates because he knows she doesn’t deserve death according to his own “beliefs” and the answering machine coming on essentially confirms this idea which contrasts her with other women. He still doesn’t have any feelings for her, he just doesn’t want to go against his own “code.”

It’s so interesting how sweet and innocent she interprets Patrick’s words. “I think if you stay, something bad will happen. I think i might hurt you. You don’t want to get hurt do you?” Jean views these words as nothing but sentimental statement relating to sexual intimacy and the potential blowback of getting involved with an unavailable man like Patrick. But Patrick’s words mean nothing sentimental. His words were a warning of her actually getting physically hurt and even wounded by what he might do. He can’t find a reason to kill her. And it angers and confuses him deeply. He’s experiencing an emotional his not so familiar with. Empathy.

Jean is the only person who is pure in Patrick's world. Jean is genuinely kind and respects herself unlike every other person in his life. She's the exception to what makes Patrick's world hell, therefore he empathizes and let's her live.

He didn't kill her because:

1. He does like her somewhat.
2. She is innocent by comparison to other people he knows (she's obviously not completely innocent, no one is)
3. She is an active, valuable and integral part of his every day life. I think they make sure to point that out at the end of the scene.
4. She shows genuine interest in him in a way that isn't vapid and superficial. Almost everyone else he interacts with is completely superficial (the other exception would be Luis; he also doesn't kill him but Luis also disarmed him with the power of gayness)

In the book, Bateman admits that Jean was the closest person he had to having feelings for. If he was going to marry anyone, it would have been her.

Christian Bale performed really well in this scene. At 4:19 when she says "Do you want me to go?", one can actually see in his face when he says "Yeah", he's actually being honest, and for a couple of seconds he's battling himself. He doesn't want her to be a victim, but he knows he can't control his urge to kill. There's a mixture of trying to hide his emotions, and a look of someone crying for help - help to take back control over his own self.

In mere seconds, Bale was able to convey all these things through a combination of facial expressions and body movements.

It wouldn’t have worked if he pulled that trigger. The fact she lived and survived made this scene more suspenseful. Even you seen this movie many times. At this scene. One can’t help but feeling the suspense and chills. Such a brilliant movie
