Many thanks for yet another very informative video. As a result I now have my nighttime BG under much better control. My worry at the moment is that having reduced meal portions and carbohydrate intake I have been losing weight for nine months. I am a 68 yr old male T1 (since age 56) injecting Levemir 20 morning/30 before bed and Rapid through the day. I was 156lbs but am now 143lbs which at 5’ 7” is still a healthy weight but I do not want to go much lower. I play two hours doubles tennis 2/3 times a week and do a hike at least once a week. I use Freestyle Libre 2 which says HBA1c is 5.8 & time in target av is 91%. Any thoughts?

Good information! not sure if you've did a video on the dangers of inaccurate CGMs and insulin stacking, but about a week ago I damn near checked up out of here, I have a CGM that monitors every 5 min so I tend to stay in range however I went off my diet and indulged in extreme sweets after about 2 hours I noticed I was still well over 300 I started taking 5 units of fast acting and dosing every 30 min to bring my numbers down, after about 2 and 1/2 hours the monitor is just staying HIGH, I then loaded up another 5 units and before my wife could say "baby use the old meter and prick your finger" I had already shot 💉 it in, , , of course I double checkd and bam the finger prick put me at 67, , I really should of gone to the hospital but thank God the lowest I got was only 48 and I was back normal by the morning, that was close just figured I should share it before someone else does what I did and wondering 🤔, why am I in this hospital or worse.

There is a perfect solution to this for all the people who it is available for. The Tandem XTslim pump together with the Dexcom sensor. Do whatever you can to let your doctor prescribe you Dexcom sensors and the next time you are allowed a new Pump go for the Tandem with IQ Control. I am steady as a rock at night, around 6.0 all night. My pump is taking care of that supported by the Dexcom. This is the pro solution in my opinion.

Tom let me tell you that you are always great. A fast question, one doctor told me that we need a certain amount of insulin per kg of weight of our body. Technically it is logic, because insulin has to cover all the cells of all over our body, isn't? When I tried this way, eat less and inject less, I got several problems ending in the hospital.. Of course eat healthy and avoid some foods is the base, even i don't do it and at the last check I was 6, 6 and I never been like that, I can be better I now.. But I am happy about it. I don't avoid northing. I admire you that you can eat so long time before to go to bed, me no, and it is not healthy my way, I know it. Personally I solved this problem since I started using the insulin that cover a long time, now I am using TRESIBA, before was another kind and similar and since I am using a sensor and this Tresiba, it is amazing, even if sometimes I may go down but not during the night but at breakfast time. If I go up I take a shot and I solve in this way. Fortunately insulin has not side effect, so I can inject extra if I need (corrections) thanks for your great video Tom, is always a pleasure to see your work.

I typically fast for nearly 23 hours each day, consuming a dinner that's rich in fiber and protein, with a modest amount of carbohydrates, between 6-7 pm. This meal doesn't cause a significant increase in my blood sugar levels; they peak at 135mg/dl post-meal and drop back to around 100mg/dl within three hours. In addition to my daily routine, I make it a point to walk approximately 4-5 km before bed and another 5 km during the day, totaling about 10 km of walking every day. Before bedtime, my blood sugar levels are usually between 95-105mg/dl. However, during the night, I notice fluctuations, with my blood sugar levels oscillating between 115 to 150 mg/dl. Upon waking, the levels are about 120-125mg/dl, which then decrease to below 100 mg/dl by noon, thanks to my fasting state. Despite this active lifestyle and dietary management, my general practitioner diagnosed me with insulin resistance, indicating a HomaIR score of 4.7. I'm puzzled by these patterns and would like to understand why this is happening.

How much protein helps blunt the blood glucose per meal?

I'm not a diabetic, I've eaten a low carb (EXTREMELY LOW processed carb) high quality whole food diet for 5 years. I'm highly active including fasted cardio and weight training 5-6 mornings/week. My weight/BMI/ hip to waist ratio etc....are ideal yet I wake up to urinate 2-4 times a night (I am 58 though) and I pretty much always wake up 4-5 hours after going to sleep (I fall to sleep within 5 minutes) and have difficulty returning to sleep. I'm pretty sure it's "non reactive" (non diabetic)" nocturnal hypoglycemia. I just don't know what's causing it. I guess that wearing a cgm might help me diagnose it but I'm not sure.

Couple of things I found from experience. If you use a tandem pump, sleep mode is a real game changer. It almost always keeps my sugar level around 120 average all night long. The other is when you eat high fat foods like pizza, digestion takes much longer so generally I have to give more insulin than usual AND give more insulin a couple of hours later. Exercising about 2 hours after you eat is also a big help. Yore asking for trouble if you eat this stuff for dinner.

My A1C is a fantastic 4.9 mmol, but I am anaemic and also take medication that interferes with result! Anyone had issues with blood pressure taken over sensor? I’ve avoided this but just thought I’d ask. Thanks for the great content you put out.
