Multiple myeloma is a type of blood cancer that affects plasma cells, a kind of white blood cell responsible for producing antibodies.

Prevalence & Incidence in India
- The incidence rate of multiple myeloma in India is approximately 1–1.5 cases per 100, 000 people annually.
- It accounts for about 1% of all cancers and around 10% of hematological malignancies.

Symptoms:
1. Bone pain, especially in the spine and ribs.
2. Fatigue due to anemia.
3. Frequent infections due to immune suppression.
4. Weight loss.
5. Excessive thirst and urination due to high calcium levels.
6. Nausea or constipation.

Signs:
1. Anemia.
2. High calcium levels (hypercalcemia).
3. Kidney dysfunction.
4. Increased susceptibility to infections.
5. Bone lesions or fractures.

Investigations:
1. Blood Tests:
- Complete blood count (CBC).
- Serum calcium levels.
- Serum protein electrophoresis (SPEP) and immunofixation electrophoresis.
- Serum free light chain assay.
2. Urine Tests:
- 24-hour urine protein electrophoresis.
3. Bone Marrow Biopsy:
- Examination of bone marrow samples for plasma cell infiltration.
4. Imaging Studies:
- X-rays, MRI, or CT scans to detect bone lesions.

Diagnostic Criteria:
- Presence of 10% or more clonal plasma cells in the bone marrow.
- Presence of a monoclonal protein in the serum or urine.
- Evidence of end-organ damage (hypercalcemia, renal insufficiency, anemia, or bone lesions), commonly referred to as CRAB criteria.

What to Look Out For:
1. Persistent bone pain, particularly in the spine or ribs.
2. Recurrent infections or prolonged fever.
3. Symptoms of anemia like fatigue and weakness.
4. Signs of hypercalcemia like excessive thirst, frequent urination, nausea, or constipation.
5. Unexplained weight loss or kidney problems.

My father recently diagonised with multiple myeloma. He was suffering from heavy pain bones. He had pains in chest, backbone, waist etc . He thought it was a normal bone fractures . So he went to Bangalore for right treatment, but diagonised with it . Luckily his kidneys are all ok. It was 1 month before of my neet 2024, we all were devastated that time.

I lost my 54 year old husband to multiple myeloma 6+ years back. We were never aware of him having this disease. The only visible symptoms were fatigue and fracture of one rib, which was wrongly diagnosed as low vitamin D by an orthopedic surgeon and he was prescribed supplement. When his condition worsened and the required tests were done and he was admitted, it was too late. I was working as a medical transcriptionist and researched quite a bit as his demise was a sudden jolt, he had stage 3. He kept complaining of pain, specifically in his legs, which I i found out later, is bone pain, as despite medicines and pain relief balms, he never had relief. How does one identify bone pain or differentiate it from muscle pain? Btw, I was told that this cancer affects the bones of the chest and spine and there are no remarkable symptoms and that through random tests, it is identified. I personally feel that these kind of diagnoses are done only by docs who are totally involved and immersed in patient care with a very sharp and alert state of mind. Fortunately, for this patient, you happen to be one of them 😊❤. God bless you!
