Best way is to take a vacation, longer is better. Atleast 2 weeks. This gets you out of your routine and away from your stress. Throw away all your cigarettes. This is the most important: quit drinking entirely for alteast 1 year. Most relapses happen after a few drinks and you arent safe from relapse at any time in your first year. Quitting smoking has to be done with quitting drinking at the same time. Lastly, eventually you need to tell yourself "I will never smoke again". For someone who smokes these are very sad words. Understand that you can never smoke again, not even 1, or all progress will be lost. Confidently tell people you quit and you will never smoke again, the more you say it outloud the more it becomes real.

Lozenges that dissolve slowly are best.
Gum is lame. Doctors should not recommend gum. It is so annoying compared to lozenges.
