1)A (Likely) ghost exits the room at 4 am each time, and somehow manages to teleport back inside the house. Or she could just enter through the back door.

2)I'm pretty sure it's referring to Christianity where people enter heaven after the Second Coming of Jesus(I'm sorry if it's wrong, I'm not Christian). I suppose the narrator's wife's soul had been taken to heaven, and the soulless wife skinned the children and propped them up while the narrator was away.

3)There were spiders in his eyes. Or his eyes were made up of spiders.

4)God gave up on our Earth and decided to try to start life again in another Solar System.
