This patient presented with vestibular migraine that responded to nortriptyline. She developed Meniere's Disease with significant hearing loss in the left ear which has been controlled with fludrocortisone. The efficacy of steroids in the inner ear persists despite blocking glucocorticoid receptors and may be due to the incidental action of glucocorticoids on mineralocorticoid receptors. Unlike glucocorticoids, most patients can take mineralocorticoids chronically without adverse effects.

I was diagnosed with SSCD because of my intermittent downbeat nystagmus. I had two TM/ MFC surgeries that did not help. I went John Hopkins and the doctor told me my DBN is coming from my Cerebellum. Since then I’ve been diagnosed with Chiari Malformation. Constant rocking vertigo is my worst symptom. It’s exacerbated by any type of pressure even standing up and sitting down. Do you often see patients diagnosed with a vestibular problem but it’s actually a central problem? Can central issues like Chiari cause rocking vertigo? All the neurosurgeons I’ve seen are honest that they don’t see rocking vertigo often. Other doctors are telling me it’s probably just a vestibular migraine. If I didn’t live all the way in Seattle I would come see you.

How long after being on fludrocortisone did symptoms improve?

Would you suspect migraine and Meniere's in someone who presents with similar symptoms except only ear fullness, no hearing loss?

My sister’s Meniere disease was complicated with so many symptoms like vertigo or dizziness, feeling of fullness in the ear or ringing, hearing loss, imbalance, motion sickness, nausea, or nystagmus and I had to gave her Dr Madida herbs from YouTube recommendation and after she using the medicine, she was with me last month telling me that she is cured completely and don’t longer experience any of the symptoms anymore.
