This patient has a lifelong history of headache and dizziness from childhood. Symptoms became continuous decades ago but received little attention from physicians. She has had a dramatic response to low-dose nortriptyline. Tricyclic antidepressants have many possible mechanisms of action in migraine patients; they are serotonergic, are medium potency calcium and sodium channel blockers, are mildly antihistaminic and are anticholinergic. They are also very well tolerated in seniors when used at low doses.

Oh, how I wish that that was all I needed! I've been tried on so many meds that didn't work, including nortriptyline. I do appreciate these videos because my neurologist and I might find a solution that we've never tried.

I have dealt with migraine and vertigo for years. Terrible daily vertigo episodes. I was put on nortriptyline and instantly was relieved of the vertigo. 2 years later still no vertigo episodes. I am on 40 mg. daily. Still some moderate headaches, but tolerable. The downside for me personally, is I was affected by a side effect of constipation from the meds slowing my nervous system down.
