As someone diagnosed with BPD I have been in therapy for YEARS. I have some insight into my condition that I'd like to share. I believe that people with BPD have impaired thinking in that the pre-frontal cortex is not working. That is one aspect. I also believe that there is an underlying disorder in patients with BPD, such as ADHD and that treatment should include stimulants if appropriate. When the thinking brain works, then people can more easily process their feelings, environment and self-regulation. In addition, think of borderlines as an energy. When their energy of freedom is stopped or disrupted or unfulfilled, they get angry. I believe it ties in to childhood abuse. Being punished for being themselves, not properly processing the trauma and being forced to be someone where they can not function, would make any person crazy. SO I believe that the disorder needs to be re-named along the lines of emotionally dysregulated, traumatic childhood with executive dysfunction. They are coming from a different place in their brains than the rest of us, and that feeling of being stuck is unbearable. The issue of abandonment is seen paired with desperation. There is desperation and emotional reactivity for 2 reasons. 1. Usually there is no (perceived) support system. 2. The skills of being able to take care of themselves are deficient and that is related to the impairment of the executive dysfunction. We need more research in this area. I believe treatment should be as serious with people in recovery with schizophrenia in the sense that they are usually followed closely during the recovery process. Borderlines need to be educated on a lot of things because their brain can not in many instances access the part of the brain that is able to understand what to do to be able to survive on their own.
