All Narcissists are both covert and overt..how they present is a ‘state’ and states shift in everyone.There are no ‘separate types’ of Narcissists..they simply present as either overt/covert dependant upon their current levels of self esteem..which ‘shifts’ their internal state.

If traits like egotism is seen as something positive, something to be proud of,
it helps me to understand why there is no will to change in extreme grandiose narcissism.

I believe we need this type of information to clarify what extreme Narcissism is about and to overcome this Narccusing.
( With 'Narccusing' I mean to swiftly and remorselessly accuse someone to be an extreme Narcissist, without taking responsibility to learn about it. )

What about those who are covert but become extremely overt during a conflict? They are frightening. Is this gear-switching typical?

So, I have a question: I read that narcissism is rare- only about 1% of the population. How could anyone actually know that? Don't narcissists think there is nothing wrong with them and mostly refuse to see a psychologist for their disorder because they believe they are perfectly fine just the way they are? How can you say there is only 1% of the population when you've also implied that they wouldn't come forward to admit their disorder in the first place? They would go undiagnosed for their entire lives unless forced to seek psychological evaluation. How often are people forced to have a psychiatric evaluation? I'm guessing it's not very common. Aren't narcissists good at pretending to be kind and generous? Aren't they supposed to be constantly keeping themselves in check to serve their needs and go undetected?
And this "questionnaire" goes against that rationale because how would a narcissist know they are behaving inappropriately if they don't actually think their behavior is inappropriate? How does this account for people who are in crisis and believe the very worst things about themselves? You can ask someone with depression, and they will find reasons to despise and punish themselves. "I served myself the casserole first instead of my aging grandmother- I must be a narcissist."
If you sub out the word "narcissist" for "vampire", do you really think the vampire is going to "out" themselves on a questionnaire? Wouldn't the survival instinct keep them from telling on themselves? You've mentioned that narcissists are incredibly intelligent especially at hiding their true nature. So how can you expect this one little question to determine with any level of accuracy whether someone truly has NPD? This all sounds like grasping at straws.

I heard a self aware narcissist on YT say that some people with NPD can have affective / emotional empathy and that this isn’t in the diagnostic criteria that they must lack empathy. I found this confusing because I just can’t see how someone gets this diagnosis with emotional empathy? I’ve never met a narcissist who has any empathy. So, just wondering about that.
