How are ketones made?!

The oft-misunderstood ketones are produced when fat burning is high in the body. Because the hormone insulin controls fat burning (by turning it down), it's no surprise that insulin effectively controls ketone production.

I am back on a keto diet for past 2months. I’m Fasting daily 12min to 14h. I might break the fast at 12h with coffee and tsp of coconut milk or ghee and actually eat first chunk of protein at 14h or even 16h . My daily carb intake is at 10 to 15% mostly from vegetables and occasionally half a cup of Berries .
When I test my fasted glucose it’s always low to very low. Like in the 60’s and Even below 60. Yet my ketone levels are always at 0.3. The only time I had ketone at 0.5 or higher my glucose was too low below 60. My glucose is never high. Might hit 100 after a meal.
I haven’t yet done longer water fasts but I did do a 3day bone broth fast where I had some bone broth for a total of about 200cal of protein and fat only daily .
Was great to cleanse my gut and give me a Good resent but I still
Could Not get into ketosis after 16h of pure fast . Or let me rephrase, I could
Not get ketone levels . I don’t know if I’m in ketosis or not…
Am I one of those cases that is just efficiently burning ketones and therefore doesn’t show?
Been checking my body composition and my % Body fat is definitely coming down so I’m Burning that extra fat storage . I just wonder if there is anything I should Or could be paying attention to anything else in particular ?

I’ve been on keto for several years now. I’m currently using a CGM to track my ketone levels. I’ve noticed that from the morning until around 5 PM, my ketones stay at about 0.3 mmol/L. Then, suddenly, they jump to 0.7 or even 0.9 mmol/L, and later drop again. My carbohydrate intake is around 30–50 grams per day. I eat two meals a day. Does this mean that my body is using ketones so efficiently that there aren’t many left in my blood?

Ketones 7.6 and BG 11.7. Measured in hospital when in for hermia op. Why is BG high but was unable to eat for 6 days, plus being sick. Dropped 14lb too.

Dr. Ben could you please explain why when we eat butter and no carbs ketones go up ? Could it be because of butter ?
