I have big big problem at work. I've worked 2 jobs, the first is in accounting firm, second is in programming firm. But thing I do and the work I submit every time just has so many mistakes even after I've checked my work for so many times. The first time, I think: "well, ok, I learn from it", second time:"I write down what I need to do to improve and execute", third time:"rest a bit, think more".... still making some brain-dead mistakes for endless time. I am really really bogged in my life point. All my hard working seems to be in vain. I really need a mentor to help.

Mistakes are usually cause by assumetions. If you repeat a mistake it is because you don't change the assumetion. We assume we are seeing the whole picture when we are not.

We need to distinguish between mistakes and bad outcomes. Mistakes might conceivably be avoidable, but outcomes cannot be ensured to be good no matter what one does. No one can predict every outcome in advance, or how to achieve a positive outcome.
