I might suggest letting your blood pressure be your guide. If you’re injecting so much that your BP is high, (with BP meds). Lower your dose, split your dose or what ever it takes. After you adjust, try increasing as you go to build a protocol that works for you.

Dr. Rand states that all we have to do is give anastrozole if E2 increases due to once per week injections. So, let me state my experience as a medical professional in integrative functional medicine that specializes in TRT for the last 3 decades. The reason someone may feel better about 50% of the time when doing once per week injections vs every other day or more frequent injections is because of dosing. For example, you give someone 200mg of testosterone and you drive their total T through the roof, lower their SHBG which in turn drives up free T and E2. The patient feels great for a few days and then they do it all over again so they never really feel bad. This protocol may be okay for someone that is healthy and fairly young (not over 50) or so. However, in a patient that is older, you end up with super elevated DHT which can cause major problems with the prostate and significant hair loss, and even acne for some. You also end up with a significant increase in RBC count and hemoglobin/hematocrit levels.

The idea is to mimic what the body naturally produces to avoid over conversion in the first place. If you don't feel as good doing more frequent injections it's because you need to either inject more testosterone per injection OR you have other hormones that are low such as DHEA-S and Pregnenolone. Check your SHBG and dose/inject accordingly. If SHBG is high and you are just starting out on TRT then you can dose high once per week and then taper off to EOD or even daily. If SHBG is low and free T is above the range, then you most definitely need to be dosing more frequently, especially if you are overweight or have some liver issues and have problems with conversion of too much T to E2 due to aromatization.

Also, estrogen is protective, so knocking it down with AIs will cause your HDL to plummet, cholesterol to increase, along with joint and muscle aches. Another thing is that estrogen helps your body get rid of old blood cells which also helps to keep RBC count in range. Also, if you donate blood you will in turn crash your ferritin levels over time which then results in lower iron levels. This causes your body to release more erythropoietin which in turn increases RBCs. So, if you have elevated RBCs, hemoglobin, and hematocrit, the thing to do is check your ferritin and if lower than 30, supplement with Ferrochel. You can also sleep on grounded sheets which in turn decreases hematocrit and improves blood circulation by reducing RBC aggregation. Taking supplements like Nattokinase and Pycnogenol also work out great especially if you are prone to DVT and your RBC count is in the upper end of normal or out of range. A lot more to this, but just wanted to point out some things.

Completely disagree, splitting dose 2x weekly, every 3.5 days I feel entirely better than a 1x a week massive blast of test that starts to waiver as the week winds down and I approach my next shot;
Twice a week I feel way more “balanced”
It also requires less AI use at least on my behalf.

A lot of guys say that by day 6 after injection they’re feeling down. Also, you skimmed over the biggest argument guys have, and that eliminating the need for an aromatase inhibitor.

I've been on trt for 15 years. Started at 100mg once a week. For the past 10 years I've been pinning twice a week. I felt no difference, but my crit and hemoglobin went back to normal levels. My test ranges for 700 - 900. Please remember "free testosterone " is what matters. My free is in the 20's.

Not really selling the idea of once a week injections is he. "Yeh we will just throw in more drugs to counteract estrogen". No thanks

I have experienced exactly what this doctor is warning about and he is absolutely correct in my case. I cut my dose in half but started injecting it twice as often and here is what happened...
1) My libido went down
2) My morning erections ceased happening.
3) My SHBG went up
4) My free testosterone went down
All of these things were negative consequences and so I returned to once a week 200mg dosing and everything improved again.

Simply different for everyone. Why do you think he said it helps 50% of the time? That means the other 50% works for them also. It's just a process that needs to be figured out for each individual. And saying that you don't need AI is BS. Some people can get away with not using them but others really need them. There is no one-size-fits-all when doing TRT.

I started off on once a week injections but then followed the new guidance and switched to twice a week, then every other day, every third day, even every day in an effort to keep hematocrit and estradiol under control. Guess what -nothing worked for that – I always wound up having to take an AI to bring down estrogen and my hematocrit always ran over 51. I recently switched back to once a week injections because I just felt better on it

I don't know anyone that's had a negative result from 2x injections a week. I know several people that have had negative results from a single injections (sensitive nipples, sluggishness on days 5-7, etc).

I'm doing test cyp every day micro administrations, feel the best this way I don't need any aromatase inhibitor

I respect all opinions here. Personally, knowing the half life of a singular injection I like twice a week. Funny I do suffer from OCD too. I do right side Sunday and left side Wednesday.

First of all the dose and thus the frequency of pinning depends on the test and ester you are using. Its going to be different for enanthate and propionate. Test enanthate which is 250mg/ml (180mg actually, rest is the ester, ) will be totally different than test prop which could be lets say 100mg per ml (80mg actual test, rest 20 ester). So if you want around 120mg per week and you are using Enanthate then you ll split 1ml in about 3 equal doses (180/3=60) and use just 2 doses during the week. I dont really know what works best though. One shot with the whole dose or two split shots? I am really concerned about this issue. It appears that its better to split it because of the half life of the drug. Enanthate half life is 4.5 days, so i believe its better to shoot every 4 days so it goes up again and not every 10 days or even worst 14 days.

I've been on TRT at 1 cc a week 200 mg, of testosterone cypionate for 9 years now. I do one shot a week and I feel great My sex drive is good I wake up at wood everyday and get wood for no reason sometimes four five six times a day or more. My energy levels are great and that's at 58 years young. But everybody's different but like I said one shot a week and I'm good, good luck to everyone and God bless you doctor.

My blood pressure spiked the first few days of my TRT, I had to break it up 2x a week to keep normal BP levels

Tried numerous protocols. SubQ ED and EOD was without a doubt the worst. Twice a week IM is perfect, for me.
