Doctors where I live automatically perform fusion bcz that's all they know and Medicare won't pay for better surgery. I've learned soooo much from you, so I have more choices. A million thanks!!

Is it common for pain to return to patients after a few years? I had three herniated discs, and I have scoliosis. I had this surgery done in 2021 and i had immense relief. Unfortunately within two years I noticed pain beginning to return in my lower back. Now I have severe arthritis like pain in my lower back but xrays show no more herniation. I'm really worried that I'm going to live with this pain everyday for the rest of my life, and I'm still in my 20's..

At what point is surgery considered??? Right now my mri indicates I have severe foraminal stenosis on my left C4 C5 and Iโ€™m feeling radiating pain that goes down to my arms as well as pins and needles sensations from time to time. Mri indicates there are bulging disc that are pressing against the thecal sac. In addition to that I also have narrowing in my lumbar with 6mm and 8mm disc extrusions that are also pressing against the thecal sac affecting nerve roots. My Lumbar symptoms are similar to my neck sharp back pain that radiates down to my legs and bottom of my feet that causes pain and pins and needles sensations from time to time. Iโ€™ve had steroid injections in my neck a yr ago and a RFA in my neck a yr ago that was effective. However this time around the RFA which I had in my neck a month ago wasnโ€™t as effective and now Iโ€™m going to get another steroid injection in my neck on Sept 16th. My Lumbar Iโ€™ve had steroid injections in the past and did traction with a chiropractor seemed to help for awhile now the pain has returned. My question is when would surgery be considered??? Does the bulging disc have to be causing spinal cord compression to even consider surgery?

Would this work for spondyliosethesis? Wouldn't it also weaken the joint? I need it done but fear it would ruin my gym life at age 35. I'm very strong. But the pain is debilitating.

Total laminectomy for decompression of the spinal canal is a outdated procedure. Single-sided hemi- laminotomy to remove bilateral ligamentum flavum and spurs with preservation of spinal process and interspinous ligament yields better result because of least destruction of spine structures.
