What is the difference?
Laminectomy and laminotomy are both surgical procedures performed on the spine to relieve pressure on the spinal cord or nerve roots. They are often used to treat conditions like spinal stenosis or herniated discs. While they share similarities, there are key differences between the two procedures:

Laminectomy:
A laminectomy is a more extensive spinal surgery. It involves the removal of the entire lamina, which is a flat piece of bone at the back of the vertebra, covering the spinal canal.
The removal of the entire lamina provides greater access to the spinal canal and allows for more decompression. It is generally used when there is a significant compression of the spinal cord or nerve roots. Laminectomy may result in greater instability of the spine, and spinal fusion is sometimes performed to stabilize the area.

Laminotomy:
A laminotomy is a more conservative procedure, usually done to relieve pressure of one or two nerves (disc herniation). It involves the removal of only a portion of the lamina, creating a small window or opening in the bone to access the spinal canal. A laminotomy is less invasive compared to laminectomy, which means it typically results in less damage to the spinal structures and a quicker recovery. It is often used when the compression on the spinal cord or nerves is mild to moderate.

In summary, the primary difference between laminectomy and laminotomy lies in the extent of bone removal. Laminectomy removes the entire lamina, providing more extensive access to the spinal canal, while laminotomy removes only a portion of the lamina, offering a more limited view and decompression. The choice between these procedures depends on the specific condition, the severity of compression, and the goals of the surgery, and it is typically made by a spine surgeon after a thorough evaluation of the patient's condition.

For spinal surgery, if your first opinion was from an orthopedic surgeon, get your second opinion from a neurosurgeon. For me, the first recommendation was for a laminectomy and fusion, and the second was for just a laminotomy which we went with. I had bone spurs on the inside of the right side of L4 and L5. He made one incision and then drilled two holes to drill and suction out the bone spurs, relieving the compression of the spinal cord. Outpatient, no physical therapy required. I'm so grateful for the advice I got.

Great! I had a successful L5-S1 laminotomy and diskectomy over 25 years ago, and I wish this video had been available then. My surgeon was skilled and helpful, but there's nothing like seeing the actual procedure (on a plastic model) to understand what the operation does. Clear explanation too. Thanks!

Thank you Dr. Webb I had a Hemi-Laminectomy and micro disc with some local spur gardening in 1989 by an amazing surgeon. He was a very highly skilled, Head of Neurosurgery and as I recall at the time I thought his fingers were like sausages that could be a challenge for him to perform. techniques Well not a problem, he did an excellent and I am doing okay today with some lumbar back aching but doing fine. I was in hospital for 3 days and flat on my back for about 4 weeks. In contrast, my sister had a similar procedure recently and was in hospital overnight and up and about the next day starting PT in a just few days. MIS access and other new techniques are impressive Thank you for sharing. God bless.

I’m having this procedure in ten days!! This will be my fourth surgery since 2019. Of course, my three prior surgeries were on different areas of my body. I’m a tad nervous to have spine surgery. But more than anything, I’m looking forward to the relief I will get post-op. I appreciate the well informed video!

I'm 1 month post laminotomy/scs revision, and having a better understanding of the procedure really helped with some of the anxiety leading up to the procedure. Thanks for making this videos Dr. Webb

Just had laminectomy at L4/5 and L3/4 after previous procedure at L5/S1 a few years ago. The second procedure was way more painful than the first. But I am hopeful that my worst symptoms will fade over time. I appreciate and am grateful to medical science and its dedicated practitioners. Thanks for the informative video!
