Please join me in welcoming Dr. Ian Lake! Dr. Lake is a medical doctor and type 1 diabetic who ran 5 marathons while fasting for 5 days. He ran a total of 100 miles in 5 days! Dr. Lake's main goal was to show how nutritional ketosis can be safe for type 1 diabetics.

Keep in mind that to accomplish this dangerous and amazing feat, Dr. Lake trained his body in many different ways. Taking something like this lightly could have been deadly.

A type 1 diabetic can not produce insulin in enough volume to maintain blood sugar. This is very different from type 2 diabetes. People with type 2 diabetes have more than enough insulin and often have too much, leading to insulin resistance.

Dr. Lake explains that our bodies can multifuel, which means we can use sugars and fats for energy. In an ideal situation, our bodies can flip between whatever fuel they need. To burn fat as fuel, your insulin has to be low.

Dr. Lake lost over a pound of fat per day during his 5-day fast. To prepare for a longer fast, you have to become fat-adapted. You must lower insulin to adapt your body to running on fat fuel. To lower insulin, lower the food that stimulates insulin—carbohydrates.

Diabetic ketoacidosis is a dangerous condition associated with type 1 diabetes. Ketoacidosis occurs when sugar in the blood can not get inside of the cells, starving them of energy. Some medical professionals believe that ketosis can put you at risk for ketoacidosis, but other doctors are against this idea.

Dr. Lake currently follows the keto diet, leaning more towards the carnivore diet. His diet consists of about 100 grams of protein per day, dairy, and some leafy greens.

Cut out all sugar, only eating fruits and vegetables, and Grass fed beef. One meal a day I have lost 25 pounds in 45 days.

I started at 234 pounds… I now weigh 147. I cut all forms of sugar, I cut bread, rice, oatmeal, all carbs except veggies… I intermittent fast ( 16-8) but I ONLY EAT TWO MEALS WITHIN MY EATING WINDOW and I DO NOT SNACK BETWEEN MEALS. And I make sure BOTH MEALS are at a CALORIC DEFICIT… I also walk 4 days a week for 40 minutes. I make sure my meals are salted liberally and I drink a lot of herbal teas with a tiny bit of stevia. I also play a lot with my fasting window. Sometimes I do OMAD if I’m not hungry too. Thank you Dr. berg.

00:00 🏃 Dr. Ian Lake, a type 1 diabetic, fasted for 5 days and ran five marathons, aiming to demonstrate extreme ketosis and fat adaptation.
02:29 🏃‍♂ To ensure deep ketosis, they aimed to burn around 20, 000 calories in 5 days through extensive physical activity.
05:47 🔄 Type 1 diabetics need to inject insulin, hindering fat burning; adapting to fat or ketone usage requires lowering insulin levels.
08:56 🍽 Nutritional ketosis involves reducing carbohydrates, around 30-50g per day, forcing the body to burn fat and produce ketones.
09:38 ⚖ During the experiment, participants lost almost 7 pounds of fat in 5 days, showcasing rapid weight loss under extreme ketosis.
11:42 🔄 Fasting and lowering insulin sensitivity are effective ways to transition into ketosis, with hydration and electrolyte management crucial.
14:00 🏃 Dr. Lake's personal transition to keto-adaptation involved gradually realizing the body's ability to use stored fat for energy during physical activity.
16:48 🚑 Diabetic ketoacidosis (DKA) is a severe type 1 diabetes complication; nutritional ketosis does not pose the same risk and is distinct from DKA.
18:39 ⏰ Fasting becomes easier with time; initial challenges are psychological, and subsequent fasts are more manageable with reduced hunger.
20:11 🍽 Exercise while fasting is easier when keto-adapted; the body becomes efficient in utilizing stored fat for energy during physical activity.
20:26 🏃 Fasting and exercising can go hand in hand, utilizing the body's fat stores for energy, approximately 85, 000 calories, making shorter fasts feasible.
21:22 🚫 Extended fasting beyond 5 days may raise concerns about potential protein breakdown, emphasizing the need for caution and monitoring to avoid muscle mass loss.
22:30 🔄 Regular testing of ketone levels, with values typically higher during fasting or after exercise, contributes to demonstrating nutritional ketosis and the body's reliance on fat for fuel.
24:21 🏋‍♂ Participants in a 5-day fasted run experienced an absence of soreness and injuries, suggesting the anti-inflammatory effects of ketones may aid in sport-related recovery.
27:08 🏃‍♂ Overcoming the mindset of needing to fuel before physical activity is liberating, especially for individuals with type 1 diabetes, allowing spontaneity in exercise without extensive preparation.
28:46 🍝 Contrast between conventional carb-fueled sports nutrition and ketogenic approach, with keto eliminating the need for excessive carbohydrate intake, reducing digestive issues during exercise.
29:24 🥩 Current dietary preference leans towards a more carnivore-based approach, including green leafy vegetables, fish, dairy, and avoiding seed oils, sugar, and excessive carb intake.
31:23 🍷 Personal experience reflects a shift away from alcohol consumption, citing decreased tolerance and reduced significance in the overall diet compared to previous years.

When I started intermittent fasting about a year and a half ago, I began fasting for 24 hours. In addition to the benefits of weight loss, I found the most profound difference with my mental health. I found I was able to focus better, I was more calm, and My overall sense of well-being increased significantly. Fasting is not for weight loss alone.

This reminds me of how David Goggins was training to become a Navy Seal and he only had about 90 days to lose about 100lbs. Lots of running and minimal eating.

I have lost about 35 lbs in the last 2 months and here is what I did. I went from 3 meals to 2 meals a day. I cut out almost all carbs from my life, I eat breakfast and dinner and in breakfast I eat a 2 egg omelette sometimes with cheese and sometimes without and no bread, with a cup of tea or coffee again without any sugar and a hand full of almonds sometimes.
And in dinner I eat a little bit of carbs and I try not to go for seconds and thirds. I eat dinner earlier too like around 5PM to make that fasting window longer.

From 230 lbs I am down to 195 lbs and have never felt better, my back pain is gone and overall I feel a lot healthier and have way more energy than before. Thank you Dr. Eric for everything that you do. God Bless

9 months ago I was 67 and pre-diabetic, 6’/245 lbs., went on 20-4 fast everyday, run 10 miles and walk 10 miles per week. I still eat carbs but a more reasonable amount. I’m now 175 and back to a size 32. Fasting/exercise works!

This video is soooo refreshing! I've been a type 1 for 41 years. Thank you for explaining the huge difference between type 1 and 2. Nobody talks about fasting for type 1. I've been doing omad for several years and it has saved my life. I also do 5 and 7 day fasts frequently. I just turn my insulin pump waaaay down. After day 2 of a 5 or 7 day fast, I ride between 88- 104 all day.

Just broke into my first sustained 20/minute mile over 5 miles with a 15lbs pack weight of the season. Walking can't really be overstated on how good it is. It nukes time sure, but something about it just feels good and works well. Hoping by the end of the season I'll be able to keep the slow jogging constant so I can push to runs when needed without breaking form. Eventual goal is a marathon a day.

Dr. Berg, when I first discovered you, it was from a referral from a friend who suggested that i try keto for weight loss. I watched your videos every single day and saved bideos for a personal playlist, and i listened all through the day for support. I lost 96 pounds and have kept it off for 4 years. Over the past 4 months, though, my situation has changed, and i am struggling: every day is an uphill battle. Please consider doing daily KETO bideos again. Your information and presentation are always so rich, and I appreciate you so much! 🎉🎉🎉🎉🎉🎉🎉🎉🎉🎉🎉
