Independent Air Flight 1851 crashed into Pico Alto at an altitude of 1, 795 feet (547 meters), while the mountain's peak is 586.84 meters (1, 925.3 feet) high. This means the aircraft impacted the mountain approximately 130 feet (40 meters) below its summit.

The crash occurred due to a series of errors, including:

1. Misunderstanding the air traffic control instruction to descend to 3, 000 feet, instead descending to 2, 000 feet.
2. An incorrect barometric altimeter setting given by the trainee controller, which placed the aircraft 240 feet lower than indicated on board.
3. Failure to conduct a proper approach briefing, which could have alerted the crew to the minimum safe altitude and the presence of Pico Alto.

The Ground Proximity Warning System (GPWS) sounded for 7 seconds before impact, but the crew did not react. If they had maintained the correct altitude of 3, 000 feet as instructed, they would have cleared the mountain with sufficient margin.
