Regarding the map fragments: you can actually see the locations where to find them before you've opened the map as these small "obelisks" in the grey area.

Helpful tip: You could leave Roundtable Hub by fast traveling. Don't jump off the balcony to the 1st floor unless you want to fight an mage invasion npc. Lost so many runes trying to get out of that place.

if you want to find the pillars for the maps they are indicated on the brown undiscovered map by a black square

Clarification - has to be third site outdoors to get Melina - had a few more than that from a cave before getting the third in the open world and she didn't trigger

About finding the maps;
The location of a map in a given area is marked by an icon of the obelisk on the empty map.
