Wisdom tooth extraction is a common surgical procedure to remove one or more wisdom teeth, located at the back corners of the mouth. These are found on the top and bottom and usually appear between the ages of 17 and 25. The third molar, better known as wisdom tooth, is the last tooth to appear in humans.

In some cases there is not enough room for wisdom teeth to emerge or develop normally. Wisdom teeth do not cause problems, however, if there is not enough space for them to grow, they could push against another tooth causing not only great pain, swelling, bleeding and bad breath, but also teeth crowding to the degree of causing deformation, cavities, cysts, infections and in some cases, even the loss of the teeth closest to them. This is called an impacted wisdom tooth.

If one or more wisdom teeth cause problems, the specialist will send the patient for a panoramic, periapical or tomography x-ray to see the proximity to the nerve, size and arrangement of the roots before proceeding with the extraction.

Once the X-ray has been analyzed, the specialist will disinfect the area with an antiseptic or mouthwash and decide the type of anesthetic to apply. If the specialist decides to use Local Anesthesia, they will first place a substance on the gums to numb them and then apply one or more injections near the extraction area so that the patient does not feel pain during the procedure. The patient will remain awake the entire time.

If sedation anesthetic is applied, the dentist will administer anesthesia through an IV into your arm. Sedation inhibits the level of consciousness during the procedure, so the patient will not feel any pain and will have limited memory of the procedure. Here you will also be given local anesthesia to numb your gums.

But if the expert decides to apply General Anesthesia, this will be administered through an intravenous line in the arm and the surgical team will closely monitor the medication, breathing and temperature. Once the patient has lost consciousness, a local anesthetic will also be administered to alleviate post-operative discomfort.

To extract a wisdom tooth that has not erupted, the dentist will perform a nerve block technique to the inferior dental nerve in the area where the anesthetic will be applied. After one minute, when it has taken effect, the expert will proceed to anesthetize around the tooth with the help of an injection and once it has been verified that the area is completely numb, a small incision will be made in the gum tissue to expose the bone that covers it and visualize the wisdom tooth.

Later, contouring will be carried out at the level of the tooth, bone and then a cut in the wisdom tooth with devices called piezoelectrics, which are used to divide the tooth into parts, so that extraction is easier, a process called tooth sectioning.

Once the tooth is divided, the dentist will use an instrument called Elevator to push the tooth out and then with the help of metal tweezers, the expert will proceed to pull it until it is completely out. Once all the parts of the tooth have been extracted, the remains will be removed and the area will be cleaned.

The dentist may or may not place a filling to speed healing and prevent infection or dry socket, which can be a complication after an extraction. According to each case, the dentist will decide whether or not to sew the wound, if he does, the stitches must be removed after approximately 7 days; however, the total regeneration of the bone and tissue can take around 6 months. In some cases, the dentist will only place a gauze in the extraction area for about 25 minutes so that the bleeding area can clot and heal faster.

Wisdom tooth extraction is an outpatient procedure, however it is very important to strictly follow your dentist's instructions, take medications on time, avoid physical exertion, eat soft foods, avoid sun exposure, do not drink alcoholic beverages , do not smoke and maintain good oral hygiene. In case of swelling, the patient can apply ice to the cheek outside the surgery site, but if the discomfort or swelling persists, it is recommended to contact your dentist.

To anyone having fear having to do this surgery: I got it done and it was a piece of cake.
I had no easy teeth to remove, the two teeth in my lower jaw were laying horizontally and had to be removed surgically. During the surgery I had no pain and discomfort whatsoever and after four days post-op I had little to no swelling left. I had no pain afterwards and no discomfort. Get yourself a doctor you trust and who has good reviews and it will all be over in no time.

1 of my Wisdom Tooth was like this and required this kind of extraction, with local anesthesia the pain was 0/10, no pain at all, only a tiny bit when they do the anesthesia itself 2/10 and after that you are good.

The only thing that you will feel is pressure, a lot of it, my jaw got numb because it forces you to keep it wide open for a longer period of time but there is nothing to be afraid of. They did sew it up after and asked me to go back after 1 week to remove the thread with which they sew the hole. I had no swelling at all, and maybe the first 2-3 days were a bit difficult because you cannot eat but after that you can slowly eat again.

If you’re worried about this, don’t be! I’ve had all 4 removed at once and they were super impacted. Surgeons perform this so many times they could do it in their sleep. I recommend going for the laughing gas, and not general anesthesia since it’s cheaper and doesn’t have as much complications. Surgery lasted 30 minutes and felt like 5, and the worst part was the needle. The recovery lasted about a couple weeks.Took about 8 months for the bone to regenerate. My life has been so much better, being able to wake up in the morning without horrible pain.

For anyone getting this surgery and having anxiety about it: I got this surgery a week ago on all 4 wisdom teeth and it went really well. I got IV sedation so the surgery was basically just a nice short nap for me. I was asleep the whole time and felt nothing. The only side effect from the sedatives was dizziness that only lasted about half an hour. The pain and swelling was the worst on the 2nd day but mostly went away after that. Prescription Ibuprofen took care of the pain and the swelling went away with some help from a bag of frozen vegetables that I held to my cheeks. My surgeon was really great too, he talked to me about school and even called me the day after the surgery to make sure I was okay. And I got to eat lots of ice cream all week :)

These comments are really helping me as I have horrible horrible anxiety. I am 22 I have an extremely small mouth and just finished antibiotics for an infection in my wisdom tooth on the top left that had started poking through the gums. One dentist trip later I have been referred to an oral surgeon to have all 4 of my wisdom teeth removed (one is growing in completely horizontal and the other 3 diagonally). My consultation is in about 16 days and I couldn't be more terrified. Both of the surgery, the pain and the fact that I am afraid I won't wake up from anesthetic. But to see so many people say it's not as bad helps me. Thank you. Cause I am terrified ;;

For anyone worrying about this surgery, i had it done on two teeth 2 months ago. They only numbed my gums, so i was still awake. There is almost 0 pain during the surgery, but there is a lot of pressure. Even after the surgery i had very little pain, maybe a 2/10, because of the medication. It was annoying to try and eat however.

It’s been over 2 weeks since I got my wisdom teeth removed, easy procedure, it was over before I knew it. I haven’t been in any pain after the procedure, just slight soreness and tenderness. Just take your medication on time, ice your face to reduce swelling 15 minutes on, 15 minutes off, and just enjoy soft foods such as ice cream and puddings during the recovery process :)

For those who fear wisdom teeth extraction in general, it really isn't as bad as it seems. I've gotten my two bottom one removed a few months ago, they were at about a 45 degree angle facing the adjacent teeth. First I was put in a room and given the numbing gas, later the dentists came in and gave me injected anesthesia, that part hurt a bit but only for a second or two. Minutes later the dentists came in to do the procedure, everything was completely painless except for the pulling of the tooth, but just like the anesthesia it only hurt for a few short seconds. Then I was closed up with sutures that dissolve in less than a month. They gave me an ice pack as the anesthesia only last about an hour or so. As the anesthesia goes away it still doesn't hurt that much, while I hold the ice pack to my face my dad went to the pharmacy to pick up prescription pain relief and anti -infection. Recovery last about two weeks and I'd say the worst of it are days 1-3 but they aren't that bad when I look back. I'll probably have to get the top teeth within a few months so that'll be fun. Just felt like sharing as others did. Also, word of advice, when washing the closed wounds don't put the water spay in the wounds or you'll get an infection, my mom, and Markiplier funnily enough, learned that the hard way.

I remember the day my wisdom tooth was pulled. It was done by a female student who was still studying at the university and she told me she'd be graduating very soon. She gave me a dose of something to numb my gums and started poking my tooth. She asked if I could feel the poking and I said Yes. She proceeded to give me a 2nd dose and repeated the question. Now my gums were fully numb and she began with the extraction. I remember hearing crunching noises and she was pulling my tooth out in small pieces. When the procedure was done, she gave me a glass of orange juice I think and I said Thanks. It was very nice of her to give me refreshments. I've never had a dentist do that before. I hope she is still pulling out people's teeth.

I had this surgery recently. For anyone anxious or worried about it, don't be. I was nervous too, it was not bad at all. Recovery is very fast and I feel so much better after it. Everything is going to go perfectly, keep your head up!

Just got mine done yesterday and was freaking out, your video helped a lot. For those who are nervous, to be honest the expectation is 100% worse than the actual procedure and the recovery. I was told day 2 is the worst, but it is ok, just like if you hurt your mouth badly, or if you ever got your wisdom teeth infected, that’s the same feeling. it’s bearable and with painkillers it’s very manageable, you can chill. I was lucky with the procedure and it only took 20 minutes (local anaesthetic) for all four teeth as mine had all come out and it was just a case of pulling them out, no stitches. The injection is the worst part, but even then it’s only seconds and bearable, exactly the same as when they do injection for fillings. It’s a bit weird when they pull the lower teeth out, I was a bit afraid for my jaw, but I trusted the professionals. They held my head in place as they had to pull hard but you don’t actually feel the pain while they’re doing it. Don’t stress and go there confidently, if you need to get it done, just get it over with ❤

For anyone doing local anesthesia and is nervous, I did this yesterday and it was easier than I expected! Focus on deep breathing, relax your body (I focused on relaxing my shoulders away from my neck), and the team I had was excellent at explaining what they were doing. The hardest part was getting the numbing shots, but it wasn’t even that bad. After that, it was so easy. You got this! ❤️🦷

When my sister and I were in high school, we had our wisdom teeth out at the same time, but my sister had to go back a second time several years later to get the upper wisdom teeth removed whereas I had all four of mine out at the same time. We both went under anesthesia and didn't feel a thing. Recovery was the longest/hardest part. We ate virtually nothing but Jell-o for about a week or so as our wounds healed. But in the end, it was worth it and glad we got it done when we did. There's nothing to it.
