My sleep specialist told me if I cannot manage the CPAP he will have to barbecue my tongue from the inside or I will have to wear a dental thing, just like that. I wish he spoke like you you are so comforting to listen to

A lot of the "bad sleep apnea MMAs" seem to be around the 2-6 mm range for the mandibular osteotomy site. A lot of surgeons do advancements like that. At that point you're really better off doing some other surgery, or seeing another maxillofacial surgeon (assuming you are sufficiently recessed) who knows how to do a better job.

Surgery is always a last resort and this is a very complicated surgery with a very challenging recovery. Why not recommend surgical expansion or even simple orthopaedic expansion with dental appliances? I treat patients with those regularly and have my own as well.

Whats your opinion on bimaxillary jaw advancement (BIMAX)? Its advertised like an ultimate OSA solution. Sorry for the general question.

I think this video is not being completely truthful? There is no indication of what movements were performed? Some surgeons are conservative with the movements, and some are outright dumb, so the movements might even make sleep apnoea worse - i.e., clockwise rotation. It also doesn't tell you the degree of jaw recession as a starting point - those people with very recessed jaws are likely to improve dramatically, but those without significantly recessed jaws are likely to improve modestly. So, this means all the statistics presented need to be taken for what they are - a summary of a wide variety of surgery procedures on a wide variety of conditions. I've found this issue with most papers on this surgery - there's too much individual variation in the surgery to be able to summarise in a meaningful way. Maybe, with AI coming in, they can do some more nuanced analysis and refine the expected outcome.
