There is no explanation and no answer
The writer & director say from their own mouths they will never answer the ending to shutter island & will leave it up to the viewer to choose, Its like one of those therapists pictures you see what you see

I liked the concept of water and fire. But I want to know what you think at the end of the story? And what did DiCaprio's last dialogue "What would be worse? Die as a good man or live as a monster" mean?
