Dr. Cawley (Ben Kingsley) reveals a terrible truth to Teddy Daniels (Leonardo DiCaprio) : Teddy is actually Andrew Laeddis and has been a patient of Shutter Island for two years. One of those rare movies where you can only experience it through Teddy's perspective the first time you watch it. Any subsequent viewing will be through Dr Lester's perspective. Such a brilliant movie.

This is one of the biggest plot twists in cinema history, up until this day I refuse to believe he was a mental patient instead a private detective

It’s so cruel that we were made to side with Teddy for the entirety of the movie until this scene, dumped onto us like ice water. I kept denying the truth and hoped that Teddy was right but he was the insane one all along. Even after all the hints the movie threw at us we still didn’t get it. Scorsese is brilliant.

The ending was really sad. He still remembers everything, but fakes still being dissociated from reality, because he’d rather be lobotomized and live as a memoryless, emotionless zombie, than remember the truth.

Ben Kingsley was perfect in this film, what we took as a sinister attitude of hiding something was actually his frustration and patience wearing thin that his treatment for Teddy still isn't working, and he makes it translate over really well. By the end of the film we actually felt bad that we suspected him because we can see he really did care about his patients.

I love all the small little details and hints we got throughout this movie that you pick up on in second rewatch. The fact that the guards who were supposed to be searching for the missing patient all appeared to be bored and uninterested and are not searching. The chains in the boat at the beginning of the movie. The strange looks everyone gives Teddy/Andrew. The fact that his “partner” the doctor has a hard time taking off his holster, despite allegedly being a US Marshall who wears a gun daily. There’s so many, but the movie does such a good job of stringing us along until it’s time for the big reveal. Incredible movie, one of my all time favorites.

The film crew whenever they have to remind Leo he's just an actor.

As a schizophrenic this movie really moves me. People don’t understand how traumatic and defeating it is to understand the reality you believe is not real. It’s so hard but at the same time it’s better than not knowing the truth. Sometimes it’s hard to trust others, especially medical professionals. It’s like seeing the sky being blue your entire life to find out it’s been green this whole time

It’s kind of fun to think about Mark Ruffalo’s journey in this movie, how he gets to cosplay as a cop and run around solving pretend crimes with his buddy

To the people who say that Ben Kingsley and everybody else were evil. In the last scene when DiCaprio seemingly confirms that he has relapsed (even though he has not) by referring to Mark as his partner, Ruffalo looks at Kingsley and the German doctor to signal the same. Even though the scene is shot from afar you can see the sheer disappointment and grief in Ben Kingsley’s face. He genuinely wanted it all to work out.

The bit where he shot Dr. Lester and he wasn't hurt was awesome.
It just hammers home the whole conspiracy was just in his head.

I believe in the end, there’s a glimpse that he realizes his fate and chooses that door. I think we all would, if we came to find what our lives had become

He realizes the truth when at the end of the film he says: what's worse? Live like a monster, or die like a good man. And this is the outcome

Leo's acting with the photos is brilliant.
Adrenaline high from the outburst he just had, the truth slowly but surely sinking in, the memories slowly but surely coming back up, but still thoroughly confused... for the moment.

Leonardo should have an Oscar for this movie especially in this scene

So glad that I didn't get spoiled until the first time I watched this movie. The revelation really threw me off, never knew that the plot twist was this shocking.

Shaped my view on Mental Health during nursing school. "I have this radical idea that if treat a patient with respect, listen to him, try to understand, you just might reach him."

On my first viewing, I thought that he'd relapsed. On the second, I finally understood what the final scene means. What he was actually saying. The treatment worked. He just couldn't bear it.

I didn't see the twist coming, and that elevated this movie to one of the best of the decade.
