I used to work in IT. I noticed that a woman who had left the company was still logging on daily into a particular service. I did some investigating. She had typed up the notes for using that service many years back and had used her own name in the documentation. Subsequently every user of that system used her details. Suggesting changing it opened up a whole can of worms. It was unbelievable. And down the years I have seen it time again and again. Things done because the instructions say it must be done. Changing the grammar in a standard letter because the it was wrong. Wedging a fire door open for 5 minutes while something is lifted through. And so on. And so on. And so on.
