Tetany: A condition that is due usually to low blood calcium (hypocalcemia) and is characterized by spasms of the hands and feet, cramps, spasm of the voice box (larynx), and overactive neurological reflexes. Tetany is generally considered to result from very low calcium levels in the blood. However, tetany can also result from reduction in the ionized fraction of plasma calcium without marked hypocalcemia, as is the case in severe alkalosis (when the blood is highly alkaline).

Following are the most common causes of Tetany :
The usual cause of tetany is lack of calcium. An excess of phosphate
Under function of the parathyroid gland can lead to tetany.
Low levels of carbon dioxide cause tetany by altering the albumin binding of calcium such that the ionized (physiologically influencing) fraction of calcium is reduced; one common reason for low carbon dioxide levels is hyperventilation
Low levels of magnesium can lead to tetany
Clostridium tetani toxin, via inhibition of glycine-mediated and GABA-ergic neurotransmission, may lead to tetany
An excess of potassium in grass hay or pasture can trigger winter tetany, or grass tetany, in ruminants.
Osteomalacia and rickets due to deficiency of vitamin D

Hypocalcemia is the primary cause of tetany. Low ionized calcium levels in the extracellular fluid increase the permeability of neuronal membranes to sodium ion, causing a progressive depolarization, which increases the possibility of action potentials. This occurs because calcium ions interact with the exterior surface of sodium channels in the plasma membrane of nerve cells. When calcium ions are absent the voltage level required to open voltage gated sodium channels is significantly altered (less excitation is required).[1] If the plasma Ca2+ decreases to less than 50% of the normal value of 9.4 mg/dl, action potentials may be spontaneously generated, causing contraction of peripheral skeletal muscles. Hypocalcemia is not a term for tetany but is rather a cause of tetany.

In addition to the vomiting a person might be in a work situation where profuse sweating does occur, as in working in a kitchen, or dish cleaning room where a lot of steam is produced through various machines and boilers. Prolonged sweating in hot and/or humid conditions, will also deplete needed minerals through the skin, and cause muscle cramps and breathing issues. So I have just learned that it is called Tetany. I have been treating my muscle cramps with egg shell calcium that I make myself dissolved in a small amount of apple cider vinegar, then I add water sometimes a bit of honey for a palatable drink that I have with a slice of buttered toast for better absorption. Fats improve the absorption of minerals.

How do you know among the three causes what has specifically caused the tetany?

I have normocalcemic and normomagnesemic tetany. Sodium, phosphorus, potassium - all it's okay.
Tetany with Chvostek sing, Trousseau sign.
Attack of tetany begins after meal and after strong running and other sports.
Disease since 2002. Sometimes I had breathing problem.
Ionized calcium is low, but I don't have a hyperventilation.
ph in blood is high...

Alcohol and strong coffe stops the attack... it's weird!!!
