This happened to me. At some point a the old toenail detached, so a new one started growing underneath. Once it grows enough it can detach the old one on its own. Commonly this will lead to ingrown toenails (happened to me also)

Now I gotta forcefully rip off an infected thumb nail some how, I went to the dr to get treated for what I think is Pseudomonas aeruginosa ( green nail syndrome) she ignorantly told me it's a hematoma even though she can see its clearly green but no pus and refused me any topical treatment or antibiotics and told me to come back in 6 weeks if it's worse
