A person with stage 5 chronic kidney disease (CKD) has end stage renal disease (ESRD) with a glomerular filtration rate (GFR) of 15 ml/min or less. At this advanced stage of kidney disease, the kidneys have lost nearly all their ability to do their job effectively, and eventually, dialysis or a kidney transplant is needed to live.

CKD Symptoms Stage 5 (ESRD)
Symptoms that can occur in stage 5 chronic kidney disease include:
• Loss of appetite
• Nausea or vomiting
• Headaches
• Being tired
• Being unable to concentrate
• Itching
• Swelling, especially around the eyes and ankles
• Muscle cramps
• Tingling in hands or feet
• Changes in skin color
• Increased skin pigmentation

Because the kidneys are no longer able to remove waste and fluids from the body, toxins build up in the blood, causing an overall ill-feeling. Kidneys also have other functions they are no longer able to perform such as regulating blood pressure, producing the hormone that helps make red blood cells and activating vitamin D for healthy bones.
If you are diagnosed with stage 5 CKD, you will need to see a nephrologist immediately. This is a doctor who is trained in kidney disease, kidney dialysis, and transplant. The doctor will help you decide which treatment is best for you—hemodialysis, peritoneal dialysis (PD) or a kidney transplant—and will recommend an access for dialysis. Your nephrologist will develop your overall care plan and manage your healthcare team.

Dialysis treatments for people with stage 5 Kidney Disease (ESRD)
Most people report feeling much better once they begin dialysis. As the toxins are removed from the blood medicines replace the functions the kidneys can no longer perform, they find they can enjoy a good quality of life. There are two types of dialysis treatments for people with stage 5 kidney disease.

1. Hemodialysis: This treatment can be done in a center or in a patient’s home with assistance from a care partner. A dialysis machine removes a small amount of a patient’s blood through a man-made membrane called a dialyzer, or artificial kidney, to clean out toxins that the kidneys can no longer remove. The filtered blood is then returned to the body.

2. Peritoneal dialysis (PD): Unlike hemodialysis, PD is a needle-free treatment and a care partner is not required to help assist during treatment.

Kidney transplant is another kidney failure treatment option
If you wish to have a kidney transplant, tell your nephrologist. Your nephrologist will explain the process of how to get on a waiting list for a cadaverous kidney or how to find a living donor.

When at stage 5 CKD (ESRD), either dialysis or a kidney transplant is necessary to continue living if you are unable to improve your kidney function through diet and lifestyle changes.

My grandmother was diagnosed with stage 5 a few days ago. Her GFR is below 15. She is 92 so cant do dialysis. Your videos have helped me a lot to learn more about this.

My wife is stage 5 CKD. Unfortunately she was in the hospital for over a month. She got out. Ten days later back in the hospital. She is also recovering from a stroke. She is doing the peritoneal dialysis. Because of her stroke she can’t go on a transplant list. She is limited mobility. I feel like we are both lost over this sickness and not sure what to do. We talk to dietician and the doctor. I feel like your videos have been much helpful than a medical professional thank you.

My 16 year old had a GFR of 5 with absolutely no symptoms. He was at a wellness check and they decided to check his blood, which showed dangerously low hemoglobin. He was taken to the ER and admitted into the ICU.

Hello everyone I truly hope that all of your GFR's increase. I have stage 5 kidney failure and I was working out 5 days a week and didn't know I had kidney failure. When I found out my GFR was at 4. Yes a GFR of 4 so I was having all the symptoms of someone with kidney failure. Your diet is key. I do not drink, dont smoke, and dont eat red meat, and I only drink water. Your sodium, potassium, and phosphorus are something you need to pay attention to for the rest of your life. Three months later my GFR is at 17. So stick with the strict diet otherwise you'll probably need to get on dialysis. I wish everyone reading this the best. It is so serious.

I join a Facebook group with people on dialysis because i thought that I would have to go on dialysis. I feel very bad for them. I pray for them. I don't want to be one of them. I can’t thank you enough James for all your videos and your advice. I was born with kidney disease. I didn’t have to worry about it too much until I turned 11 years old when my autoimmune disease just damaged my kidneys even more. They told me that I would need a transplant by my 15th birthday and I just turned 29 years old and haven’t had a transplant yet. But sadly, last year in 2020, they told me just 3 days before Christmas that my GFR was at 13 and my kidneys got to stage 5. I didn’t know the symptoms of stage 5 kidney disease when I first got it. But when they showed me the list of symptoms of stage 5 kidney disease, I had every symptom on that list. If I knew what the symptoms were earlier, I wouldn’t be so surprised when they told me. But the last time they checked, my GFR went back up to 17. So all of your advice is working. So I can’t thank you enough for all of your wonderful advice and for giving us all hope. P.S. I love how you start a lot of your videos with “ kidney warriors “ because my entire family has called me the kidney disease warrior my whole life. 😊

I agree that just drinking water for fluid intake does help. Dehydration is a big one. I didn't think that also loose stools is a worry and forgot to make mum drink more. Hit the kidneys hard.
