The underboss is the guy that's more public, the intimidator, the guy that keeps people in line. The boss is the guy in the shadows, not putting himself out there to be targeted. You also have the #3 who is the consigliere or counsellor for the boss who provides 'wisdom' on important decisions. The #3 is often the most important cog to the machine. This may shed some light on who these people at the top are.
