What keeps you from sustaining more bone loss with just 4 implants on top and so much space between them?

I, m a dental tech. for 47 years.I used to do alot of crown and bridge.To me an all on 4 is a 12 unit bridge supported by 4 implants.I never considered making that type of bridge with only 4 natural teeth for support even though natural root structure is more extensive than implants.How good is the success rate in years considering if one implant fails the whole bridge fails.This is a very expensive procedure for only the few that can afford it. Is it worth it? I sent a comment a while ago but you never responded to it.MAYBE YOU'LL RESPOND TO THIS ONE!

Does plaque/food/debris get in between the upper denture and the patient's soft tissue? How do you deal with that? I have a hard time imagining the patient cleaning that with a water pick.

Do I understand correctly, you do the extractions, place the implants and load the implants the same day? My understanding is that it takes 4 to 6 months before the implants are ossified and that loading the implants any earlier is not advisable.

Hi, who should be the one who clean patients' zirconia crown on a titanium implant? Is that the job of a periodontist or the job of the dentist who did the crown/implant? Or can a hygenist do that? Does the crown have to be unscrewed for cleaning the implant? I saw some videos that seem to show that dentists have to do that? Thanks.

I had all on 4 on the top and bottom jaws .  I'm due to go back to Turkey in 2 months time to have 

the temporaries removed and have a  porcelain  fused to metal prosthetics fitted,

Can you tell my why the implants on the patients x rays are at extreme angles and not put in as vertical as possible .Simple logic would tell you that angled implants are more prone to failure. Also can you answer about failure rate due to improper dental hygiene as fixed all on fours are hard to keep clean and infection around the implants a real possibility .What is the expected life span of all on fours as they are very expensive so how long do they usually last.
