I know this seems obvious and it makes me feel very stupid, but make sure you triple check that you've scratched the cards off fully. I was having trouble trying to redeem a code for a while until I realized one of the Os was a Q, but the code font makes it very hard to see this without scratching literally all of the obscuring material off.

My problem is a little more complicated: I have a downloadable content that I already redeemed its code on ps4. Now I'm playing the game on ps5 and says that the downloadable content has to be purchased again to be in the game, but no matter how many times I write in the code, it tells me that the prepaid card could not be redeemed and that I should try it again later, and not that I can't redeem it because it has already been used. It's kinda strange.
