Just an aside. I am in peri menopause and actually a patient of doctor Hirsch. The hormonal effect on sleep is very real. But I wanted to add another wrinkle to the whole sleep thing. I didn't realize over the years that I had become more and more sensitive to caffeine. Only when I started getting little heart palpitations and got checked out did a doctor suggest that I take a break from all stimulants just to see if that would make a difference. I literally only had one small cup of black tea every day, I was not a caffeine junkie. But when I gave up caffeine for a while, the palpitations stopped immediately. What I didn't expect was a huge change in my sleep. I was back to sleeping like a 16-year-old. I periodically allow myself a cup of black tea when I'm just really missing it, and invariably I have a terrible time going to sleep that night and will wake up feeling exhausted.

So this is just another thing that women in midlife may potentially be dealing with in addition to the hormonal stuff. Apparently everyone becomes more sensitive to caffeine as they get older. Just another wrinkle to add to the mix. If you are someone struggling with sleep, maybe just try giving up caffeine for a month and see if it has any effect

At the age of 48, I adhere to a regimen of 200 milligrams of bioidentical progesterone orally, supplemented by a modest application of testosterone cream. In contemplating adjustments to my hormone replacement therapy (HRT), particularly in the context of perimenopause, I inquire about the appropriateness of introducing a minimal dosage of estrogen. In the UK and age 42. I first started noticing symptoms, irregular periods when I’d always been regular before, mood swings (not just irritability but blind RAGE for no reason or minor inconveniences) etc etc 2 years ago. Got my bloods done but GP reported levels were normal. Symptoms continued and I ended up getting a progesterone IUD. Things levelled out for a while but then around 8 months ago the insomnia started. Out of nowhere I was suddenly lying awake until 5am before dropping off for 2 hours, nerve pain and restless legs and arms. I tried a combination of things and have a prescription. Which has again been working well since April. But now the insomnia is creeping back, I’m staying awake later and later each night and I could just cry. I’m a zombie, I’m messing up at work because I just can’t concentrate, anxious and stressed ALL THE TIME and cry at the drop of a hat. I’ve done the pee test things (no idea about my cycle anymore since I don’t get periods with the IUD) but each of the 4 tests have shown a strong positive result. I know I need to go back to the Dr but I don’t want to be fobbed off with “you’re too young” like last time. Both my Nan and my mum started early too, this is standard for my family. Do you have any advice? I feel like our GP’s over here just don’t care and think it’s all in our heads.

How do I get my HRT balance right to achieve the closest thing to a pre- peri menopause reset and get rid of this sudden onset of excess stomach weight? 😩
I just want my body to process and metabolise how it did before peri menopause. This menopause mid section sitch is costing me a fortune in new clothes! ☹️🤭

I just turned 45 and can fall asleep but I can’t stay asleep. Hot flashes and night sweats are starting. I toss like a rotisserie chicken and I can’t live like this for the next ten years 😭

Patches are horrible! They need to make an adhesive that people are not allergic to. Also every time I changed the patch I got a period! That defeats the purpose of having the IUD in which also didn’t stop my regular periods when it was supposed to. I seem to be getting relief from the Hrt gel after 9 days of using it. Hopefully I’ll see more improvements over the next couple of months

Hi Dr H, Thank you for your time and videos. I started estrogen and progesterone in lowest dosage 4 weeks ago . First I used the spray I felt little better little bit more energy and happier but I scared to transfer for my cats and teen son then I switch the patch. OMG with the patches first 2 days I am so very very angry to the point I can't do it anymore and I had to switch back to spray. I spray and cover my arm 2 hours so nobody gets in but then I am having a different problem my skin is reacting badly to the bad aids (not the med) getting really red itchy and sore.. I wish they had daily patches not weekly. Do you have any suggestions for me. Thank you so much.. Ps; I am also worried I take shower or wash my arm off the hormone, can anybody still get in them, my children taking baths at the same tub..

Can you take Prometrium if you also have a hormonal IUD? Or is that too much progesterone? Also, do you need to match dosages of estradiol/progesterone? Meaning, if you get a higher amount or estradiol, are tou smart to get a higher dose of Prometrium too? To balance the two? Thanks for always giving us such great information!!

Can I take micronised progesterone 100mg (Utrogestan known in uk ) orally every night without estradiol, Is it safe please ? Any advise would be great help. Many thanks!!

Do any of you have painful HOT FLASHES!? It's in my legs and its like electricity running through them. I've told my Dr's and OBGYN and none of them know about it. I stopped having periods 5 years ago. I know exactly when because i had my last period during my mom's funeral, and I had almost gone a whole year before that. I would say the painful electricity part started the same time even though I'd been havinghot flashes for a long while. My Dr FINALLY gave me prempro about 2 years after I stopped, I was going insane from lack of sleep. It worked great until this April i started spotting on and off for about half the time for the next 5 weeks. I had to go off the estrogen until I got a Biopsy and it came back negative. The OBGYN said I could go back on prempro but I have been back to having painful electricity running through my legs. It only happens WHILE im have a hot flash. Weird huh? It should go away again once the prempro kicks back in. Also I get really cold for about an hour BEFORE a hot flash. Also unusual.

Hi Dr, hope you are fine and wel. I had Bso, hysterectomy, Appendectomy and omentectomy in Dec 2022 for high grade serous cyst adenocarcinoma both ovaries but my Dr didn't prescribe any HRT. I'm obese with 26bmi, hot flushes, increase sweating, palpitations and occasionally high blood pressure. Please advise what should it do. Regards

Dr. Hirsch, I had a partial hysterectomy 12 yrs ago and a recent unilateral oophorectomy. I began estrogen replacement therapy (transdermal patch) about 1, 5 mos ago. We're still trying to get the dose right. A lot of my symptoms got better but I still have trouble falling asleep. Would it be reasonable to augment estrogen w/ progesterone?

Mu situation is very strange.46, no symptoms apart from a belly I never had.I have never had children.I sleep like a baby 8/9 hours.I do intermittent fasting, I eat well and exercise but the belly and the has to be perimenopausal symptoms
