I was surprised the Bhopal disaster, the 1984 gas leak at the Union Carbide pesticide plant that killed 0ver 16, 000 people wasn't on the list.

Reactor 4 at Chernobyl did not simply "get out of control" of its own accord. The cooling system pumps were turned down, so the reactor over-heated, and the automatic protection systems were turned off. As you implied, operating instructions were ignored, to put it mildly.

Correction; it's a common misconception that a 1, 000 year flood is only seen once in a thousand years. It actually means the odds of it occuring in a given year is 1 in a 1, 000. It's quite possible for such a flood is occur more than once in a relatively short time

Ah yes, Chernobyl. The finest example of ‘we f***ed up’. I remember watching a docu-series on radioactive locations. One place was, naturally, Chernobyl. The Geiger Counter was chirping frantically the whole time. Until one room where firefighters’ gear had been piled. When the door opened, the Counter hiccuped, shrieked, and died. The host looked at his dead machine, and pulled the door shut. “We should leave now.” Wisely, the crew did. At a swift trot!

I would have thought that a disaster with over 26, 000 immediate deaths would rate far higher than where you put it.

I live in the Miami Area. And seeing the Surfside Collapse was tragic too all of South Florida. The local news did not stop the special coverage until 3 days after the collapse. People got together and helped the victims. During tragic times, the diverse community of South Florida united.

Correction: Natural gas is lighter than air. Propane and butane are heavier which makes a gas leak from these sources so dangerous.

With the last story on the apartment block on Miamis surfside Beach, they collapsed from developing concrete cancer from water penetrating into the pours of the concrete. The wrong grade of concrete was used and it should of had a additive put into the mix to waterproof the concrete and this disaster would of been avoided but then again it was cheaper to use the grade of concrete that they were using and that water that entered the basement would of had a saline level in it due to being right on the beach front and the basement being at or even below sea level and once that salt water penetrates into the pours of the concrete and comes in contact with the steel rebar that's just asking for trouble and this is usually a common problem that accurs with concrete wharves, jetties and especially ferro cement boats if not done correctly and I've seen it for myself enough times in my life and the ss Atlantis WW1 concrete ship is a classic example.

Places in China are so notorious for floods that everyone knows they have catastrophic floods, yet the flood plains have been heavily populated for centuries .

So number two wasn’t really a fail.

The storm was twice as severe as what the dam was designed to withstand.

We're told that officials were charged. With what and what happened to them? Were they convicted and sentenced to any penalties?
