If your controller doesnt connect back to your ps4 what you should do is go to bluetooth devices then disconnect your controller which is not connecting back and press share botton and the PS botton at the same time and you should see your controller available to be connected back its a glitch of Playstation they should fix it but this is how you fix it back :)
