In rare situations deathclaws sometimes throw cars at you. Mabey the code in the game thought you should die from that slight collision.

The car even gave you a warning growl, moved a bit and you still pogo jumped on it... never mess with cars in their natural habitat.

The Sole Survivor Twisted His Ankle While Landing And Severed An Artery In His Ankle Causing A Great Strain On His Heart Leading To The Heart Attack We See On Screen. It's all very intuitive

I’ve had that happen on a 0 death play while walking to the final quest I jumped and randomly died

It’s like the Skyrim boulder traps where if the small rocks are still moving and bump into someone’s ankles they just drop dead.

One of the first things I do with Survival is get Survival Options and give myself an Autosave object, along with re-enabling the timed autosaves. The game crashes far too often for me to accept only saving at beds.

One of my last Survival Mode attempts: I was heading through one of the downtown areas and heard some gunfight in the distance. It was some Brotherhood members with a Vertibird were fighting with a large group of Super Mutants. I quickly learned in Survival Mode that it's best to avoid most fights because they can either kill you too easily or the cost of resources (healing items/ammo) aren't usually worth the possible rewards. So I decided to avoid the entire fight and started making my way around to bypass the area entirely. Well I get about 2 blocks up the road where there is an explosion and a moment later, the wreckage of the Vertibird crashed right on top of me and killed me. I lost a few hours of progress thanks to an AI fight I was no where near a part of being.

I quit playing for several months after that before eventually jumping back in with an new attempt from scratch. Despite how brutal it can be, I cannot bring myself to play FO4 on any difficulty other than Survival Mode once I started messing around with the difficulty mode.

I ALWAYS avoid jumping on cars, not for this exact reason, but close enough.
I was running around Diamond City outdoors, when a pack of mole rats (Or dogs? I forget now) popped up, one legendary. So I jumped on the car. One of them bum rushed the car, bumped into it, and the car started to spaz out and started to levitate while I was standing on it.
It kept going higher and higher and higher. I stayed on it until the ground disappeared. Then I jumped of (100% fall damage reduction) and it took me like a solid minute to fall.
