I'd like to see more on Flipped Quaternions issues (which you briefly mentioned at the end). I am yet to understand what the conditions are. Taking the keyframe and going to Pose>Flip Quat fixes it but it's still confusing.

A bit more about the math:
Quaternion rotation doesn't say "rotate 3 axes" it says "rotate around one axis, which is pointing in an arbitrary direction". This allows a clean rotation from any point to any point without issues like gimbal lock or weird arcs. The limit is the range of rotation of course
