This is the exact logic I went through when playing Seven Days to Die for the first time. Normally works in other games. Barely knew what I was doing and dying of thirst. All out of clean water, struggling to find more. My buddy and I decided one of us would take a risk drinking from the pond. *You have contracted diarrhoea* the game prompts me. Thirst and hunger debuff. "Hmm. That's not good, but we've got plenty of food. I guess I'll just have some more. What's the worst it's going to do? Give me _double diarrhoea_?" I comment in almost those exact words. Drink drink drink...

"Oh shit. It actually did."
Friend laughs in horror as I literally bleed to death out of my arse over the next few minutes.

Fun fact: In the MMORPG Tibia, items you throw out stay on the ground and players throw less valuable loot like food out. In some areas, the ground resets only every few months.
It does not have a rotting mechanic though.
