How the Medicare Part D Deductible works

Does the deductible apply per medication? In other words, in your example, after I pay the $455.34 in January for my eliquis prescription and meet my deductible of $435, the next time I fill my eliquis prescription in February, I will only pay $35. But what about my imatinib? Let's say I don't fill that until february, after I have already spent $455.34 on my eliquis prescription in January. What do I pay for the imatinib? Is it $2220.52 because my $435 overall deductible has ben met or do I have to pay $435 for imatinib as well before the copay kicks in?

And what if I had started in January with imatinib instead of eliquis? Would I have paid $8882.08 minus $435 (or $8446.92) for my first filled prescription of imatinib? And then would my eleiquis prescription filled subsequenty (but still in January) only been $35?

Thanks in advance

This whole process is confusing including the Medicare plans like the difference between United Healthcare and straight Medicare and what Medicaid pays depending on your qualifications

If I have an expensive drug I have to buy on January 1 of the year, where I pay more than the $435 for 90 day supply, what happens to the balance of the money? I overpaid my deductible by a lot. Does the balance go into a "credit" for future prescriptions?
