Thanos looked surprised because he just realised that there was *a bit of green in the blue* of Captain's eyes.

Probably because Thanos didn’t see himself as evil. In his eyes he was doing everything to save the universe. Seeing caps life showed that cap fought evil. So Thanos questioned himself for the first time. He thought for the first time that maybe he wrong…Or he was shocked that cap was so jacked.

The way I felt the scene, Cap's eyes and effort in the face of destruction said it all, no Soul Stone shenanigans needed. Thanos realized that this man was 100% dedicated to a selfless cause, just like himself. He had not come across many such beings during all his years, so it was a shame to meet someone like that opposing and not understanding him there, at The End. He gave Stark props for similar reasons earlier.

In a deleted scene, Thanos also can be seen muttering "This guy is still a virgin??"

Although many discard Thanos as a villain, and nothing more, having read the book detailing his backstory, I can understand him better than those who have only glossed over the surface of his quest to purge the universe of half its population. There's a reason he's doing it, and I HIGHLY recommend reading the novel "Thanos: Titan Consumed" because it really illuminates the journey into the "madness" of the mad titan. Not every villain is evil, not all heroes are saintly. Cap is one of those few sterling examples of what it means to stand resolute by a moral code, no matter the cost, and THAT is what made the titan pause... having to be opposed to such a force and possibly have to destroy it for his own cause, Thanos looked like he wavered a bit; regret is what I saw in that instant. As in, he regretted that Cap was so directly in opposition, an immovable object to his unstoppable force. One of them had to give, and Thanos hated the necessity of putting down Cap, IMHO.

This was such a powerful scene, you can see in his eyes he really really cared about mankind and was giving every last breath and effort in his body to save it. He did such an amazing job with this scene.

If the stone had been in any way involved at that moment, then it would have been glowing. Thanos already knows about the avengers. He knew Stark by name, and it's safe to assume that he would have known all the big players and their history. Instead, I think this was a moment in which Thanos couldn't comprehend why they were fighting him so hard. He was inevitable. He walked right through earth's mightiest heroes, but here's this guy literally putting himself in the position to be completely destroyed to stop Thanos from "Saving the Universe". I think the look is pity, because he views them as lower, less intelligent life forms who are going to lose to him because they can't understand the benevolence of his actions.

The movie was always about Tony and Steve. Tony showed him early on and cap showed him here. Tony was the one to make the sacrificial play and cap being more than a special bottle... worthy

Thanos's code is he respects warriors. Gamora's death hurt him, because he raised her to be his greatest warrior. This is why he respected starlord, because he took the shot on Gamora. This is why he collected many types of armies. And why he didn't kill hulk, drax, mantis, black panther, starlord, Tony, etc.. Anyone who stepped up to fight, he let live until the snap. That's why he killed loki. Because he was a weasel, backstabbing, shape-shifting coward.

I think this scene really shows a connection between Thanos and Cap. As he's able to truly look into him he seems less surprised and more like he's processing and understanding him much deeper. I never actually paid much mind to the expression but it definitely stands out now that it's pointed out to me. It just seems to contrast the moment as strange and intuitive, you wouldn't expect someone to make that expression during a struggle. At least, in my opinion.

Plus the fact that Thanos believed his own actions to be just for the greater good, so when he saw how noble Steve is with his entire life, he was in genuine awe that a simple human was this devoted.

I wonder if he also saw himself through Cap's eyes. Thanos thinks he's doing the right thing. Steve Rogers sees him as another version of Hitler.

Makes me wonder, what if the ‘Snap’ wasn’t 100% random?
Iron man and Rogers both survived, two men he respected.
