The guy running his finger across his arm is inducing a stretch reflex in the muscle fiber to when you quickly change ita orientation, it bunches up. The way your muscles respond to that stimulus are very localized possible a larger number of stretch receptor nerve endings in a smaller area instead of the opposite. A genetic expression where your muscle cells respond that way from surrounding nerve fibers.

I am a doctor and you stumped me. I can only speculate that this is some form of severe pitting edema. It seems like there’s a great deal of subcutaneous fluid in his upper arm. There are no large ringed structures like retinaculum along the upper arm that would normally herniate or bulge like that. I’d love to find out if anyone knows!
