The intervention is definitely LIFESAVING. That said: if you don’t resolve the “how you got there” from inflammation, food, glycation, carb/sugar you will be there, again.

I asked my former Cardiologist if he could tell from the tests that they had done to identify plaque whether it was soft or not. He said "I don't know what you are talking about".

Yup, fluid dynamic, a point blockage of a length less than the diameter of the artery will have greater fluid flow percentage (what I think you are calling FFR) than the total blockage percentage at the maximum point of obstruction cross-section area. As the blockage's length gets longer than the diameter of artery, the flow blockage percent should approach (increase) to the blockage area percent.

I had 2 major stents 12 years ago, then a penta-bypass 4 years. The statins caused me periphal neuropathy with kidney complications. Dietary changes is key to recovery. I do appreciate the medical field for truly saving my life, in spite of the complications of certain medicines, considering it was my own self-harm. Presently I am slowly recouping, but definitely working, keeping up with the current science of supplements. Thank you Dr. Brewer for sharing these narratives!

My understanding is that "in general" cardiologists only place stents where there is a flow limiting plaque. In view of this study will cardiologist now have a case to stent every location where there is any plaque regardless of its type and effect on flow? (Maybe a good time to invest in the stent manufacturers?). My guess is that the majority (but not all) of the flow limiting plaques would be calcified. It would seem that the rationale for this study is a bit puzzling.

Overall very good content. I take a small issue with the assertion at 45:35 that if you are eating high carb you will have triglycerides over 100. That may be true for processed carbs, but I'm eating whole food carbs around 230 g/day net carbs and I got my triglycerides down under 80 and improving all my metabolic measurements by lowering saturated fat in my diet.

The problem with most of these studies is that it is all designed as hypothesis testing. It is absolutely impossible to control all of the possible variables. That’s why absolute risk “associations” are not convincing unless they are large. We need more studies funded outside of industry like the citizen science foundation. Possible tax breaks for donations to this type of study would make sense.

my question is why cant they use a Atherectomy or a Turbohawk Catheter and after that procedure why put a Stent?

I am a little bit confused. Dr Brewer has been doing these videos about stenting for a while now.
Is he doubting the benefit of stents for ANY type of blockage, or only those that are not meaningfully blocking arteries ?
For example, I got a stent a year ago after a CT scan angiogram+OCT revealed that my LAD was blocked at 70% in one particular location.
In DrBrewer's mind, does this belong to the type of stent that he would call superfluous ?

Are You saying that everybody with a high Calcium Score should undergo OCT /US?? I surely hope not! I am 81. I had a Coronary Artery Calcium Score of 400 some 25 years ago.

To address the last question, if it were me, I'll probably get a CTA with AI interpretation, and If I was dealing with soft plaque and chest pain I'll probably get a stent, Orbita 2 trial shows evidence of improvement of angina

Is the placing of a stent a dangerous or risky thing? I have angina pectoris and in the Netherlands they are not eager to place stents.
