Let's talk about insulin resistance and gluconeogenesis. Gluconeogenesis is the new creation of glucose made by your liver. It's stimulated by a hormone called glucagon, and it's inhibited by a hormone called insulin. Insulin is triggered by glucose from the diet.

With gluconeogenesis, your body is making new sugar out of non-carbohydrate sources. Your body makes this sugar from protein, fat, and ketones.

This happens because your body does need a certain amount of sugar. However, the sugar your body needs does not need to come from your diet. Your body can make it from other sources.

Insulin resistance is a precursor for prediabetes. You'll have insulin resistance if you have prediabetes or diabetes. Diabetes and prediabetes are both high sugar situations. A high-carb diet can contribute to the situation, making it worse.

But, when someone starts to cut carbs out of their diet, sometimes their sugar is still too high. How can this happen if they're not eating any sugar? Well, it must be coming from gluconeogenesis.

The body is making more sugar because the "off switch" (insulin) is not working. You don't have enough insulin because your insulin resistance is so strong. You have so much resistance that the insulin is not able to get through. Your body senses a deficiency of insulin, and it will make more sugar.

If you're in this situation, you need to stay on the keto diet and keep doing intermittent fasting—just give it more time. Eventually, the insulin resistance will improve to the point where your liver won't keep making excess sugar.

It took 2 years for my blood sugar to fall under 99. It hovered around 105-110 for a year after I lost 85 pounds which, at the time, was frustrating. Now it’s hangs out 80-90. Thanks Doc for keeping me informed.

I'd add that in addition to removing carbs, you also want to do other things to improve insulin sensitivity, such as:

• Get better sleep
• Address stress
• Take ACV before meals and while fasted
• Keep electrolytes topped
• Supplement (there are lots of herbs that are said to help with insulin sensitivity)
• Do resistance training
• Do Light Cardio (like walking This burns off the glycogen created by the carbs and gluconeogenesis, whereas heavy cardio can increase stress.)

Perfect timing my wife eliminated all carbs but her blood sugar kept going up then she would medicate.
Now with this new information we can make strategies.

This was helpful. I'm a 53 yr old male, 230lbs, walk 3-5 miles 6 days a week, lift weights 1 time per week, disabled, eat very healthy(carbs under 20 per day, no sugar for 1 1/2 yrs, very low proteins as I mostly eat salad and cheese- have to take B12 because that's low). I am doing everything to get off insulin and it's under control with only 20 Levemir and 12 Novolog per day, but I still will see my BS rise above 180 if I don't use the insulin. I bet its Gluconeogenesis. That makes sense. I sometimes feel discouraged but will keep at it after understanding this. No Doctor has explained this before and always look at me as if I'm not counting hidden carbs or I'm lying. Thank you I only eat from 12pm to 6 pm and was feeling tortured.

Awesome video! I have run into so many diabetics and pre-diabetics who have given up on keto because they said it didn't lower their blood sugars. Great stuff! Your video 2day on gluco-neo-genesis was great! Your graphics...wow...excellent! Must say your combo of very important subject (diabetes), high processed foods=overload of sugar=overload of insulin=burnout of pancreas insulin making=Liver making glucose=massive sugar overload=Diabetes.
All this voice-over information while beautiful 3D graphics rolls onward in a 5 minute sound-byte. Bingo! I think you have the perfect way to help educate we YT armchair viewers about epidemic size health problems (international scope) of Diabetes & related Metabolic disorders...obesity and others. Am watching you & Robert Lustig MD, Endocrinologist, who has many YT vids on the interworkings of our organs on coping with impact of highly processed foods containing tons of sugar. great video, that would explain higher than normal sugar levels for some time even when on the keto diet, it just takes a long time to lower that insulin resistance ;)

My glucose always goes up for the first couple days of a prolonged fast. It usually comes down after 2 days.

THANK YOU DR BERG for this video. I couldn't understand why after 12 days doing Keto - basically zero carbs - my blood sugar went up in the mornings. I felt very frustrated and wanted to give up. I've been pre-diabetic for years and now understand that I am insulin resistant. Now I know what to work on . Ive searched for answers and this video clearly explained what was happening in my body. I am one day from hitting the 6 week mark in Keto, being an older male, 66, I notice the weight loss is not so prominent, just have to be patient, the body will change at any age! Blood sugar numbers are getting better, not perfect but I can see the Am Glucose spike is at an acceptable level, below 185. A month ago, it was 230 at times this during the am glucose spike.
