So, what is salvation and who determines whether a person is saved or not?
First off, the determiner for eternal salvation is always God and always will be God, not any created being.
Salvation, from this standpoint, is freedom from the judgement of God.
Salvation, from a human standpoint, is freedom from impending doom in whatever form that it takes.
Lifeguards do it, paramedics do it, doctors do it, soldiers do it, etc. on the physical plane of existence though limited to only physical health and safety.
Then is salvation different when God does it on the spiritual plane of existence? Yes and No.
Yes, it is different, in that God has bound Himself to certain standards that He may not and cannot abrogate and still be just and the justifier of those who He has chosen to save. (Romans 3 and Ephesians 2:8-9)
Sin against a holy God must be paid for, otherwise God, the righteous judge, could be called unjust and that could never ever be.
No, it is not different, in that God, like the rescuers above, has the power to affect changes to any physical situation besides having the power to affect changes to any spiritual situation.
There is no difference from being saved from physical doom as there is being saved from spiritual doom. When you are saved you are saved.
Though we cannot save ourselves from the impending doom of Hell because, as in the examples above, we are powerless to change the situation. It takes someone outside the situation and with the ability and power to change the situation to act in such a manner as to actually change the situation. The impending doom, at that point in time, is taken away and we are free from it's power over us.
So, is a person saved forever or still dead in his/her trespasses and sins forever from a spiritual standpoint?
Many think they are saved by what "they" have done, but salvation is not something that we accomplish in our own righteousness (as we have no righteousness in and of ourselves) but is something that only God accomplishes in His righteousness.
Like God Himself salvation is both simple and complex.
Simple in that there is only one way to God - Jesus. (John 14:6)
Complex in that there are many different paths to Jesus, since we are all individuals with different strengths and weaknesses that make us who we are, although all of them end by giving glory to God and God alone.

Yes they can. Otherwise the Holy Spirit would not have mentioned it through the Apostle Paul. Stay away from the Calvinistic doctrine. It will damn your soul to hell.

​ No one can snatch them out of his hands, but they can leave willingly. They can be seduced. It is the believers who are deceived.
What about this, ..Someone comes to believe in Jesus Christ. Then comes the devil and seduces them into believing that Jesus Christ, is actually a word called Yeshuah, and now people pray to God in the name of Yeshua?
Or someone comes to believe in Jesus Christ, and understands that God is the Father. Then comes the devil and convinces them that the Father is actually a god named Yahweh?
Or someone comes to believe in Jesus Christ, and here comes the devils "Ministers of Righteousness". And they convince the new believer that he must only trust in the original or oldest and most extant manuscripts in order to know truth, causing the new believer to latch onto scholars and wise men and pastors to guide him into truth, other than trusting in God and God alone?
Or someone comes to believe in Jesus Christ, and then here comes the devil to convince them that Jesus is God the Father, God the Son and God the Holy Spirit all rolled into one, i.e. Trinity, therefore believing in a "Strange God" and denying the Lord that bought them? Yes that is a strange god. There is no man who is created in the image of God, who is his own father. There is no man created in the image of God who is his own son. That would be strange. Yet, that is what is taught.
What if someone comes to believe in Jesus Christ, and then here comes the devil and convinces them that they must trust in man or even themselves to determine what is the truth, and they go off seeking wisdom from doctrines and precepts of men such as the Septuigent which cause confusion, and then they end up being one, who is actually helping the false prophet, with his false doctrines, leading others astray?
The "Once saved, always saved " theme seems to have alot of holes in it.
What about those churches of Revelation? What does the Spirit say unto the churches and why must they repent? What happens if they dont?

No Christians can't fall away. Sproul only will teach calvinist lies with him trying to thin out the crowd going to Heaven.
