At 1-41secs there is a schedule of those parameters (which after editing for your particular circumstance) could easily represent the basis from which you can create your own advance health care directive. Sadly for me, there was no sound, but I think I got most of it from the subtitles. An extraordinary valuable mini-educational. Thank you for posting

I wish doctors would respect what patients want at all times, not just at death.

Since I have no family, and I am pretty much open to a full court press of treatment I haven't made an advance directive. But if I showed up at the ED, a John Doe, I probably should make it evident that there are no relatives or friends to seek advice from. I do continue to have a medic alert bracelet of being a kidney transplant candidate around my bill fold wallet. I've had a patent renal graft since 2012. I would still need leukopoor irradiated blood products. My AV fistula shows no sign of having been cannulated for years. Still battle axe nurses* will say I am a professional patient, not a compliment. As opposed to doctors who note that I am a good historian. After the nurse passed on word I was back in sinus I had a 3rd lifetime cardioversion 2 days later at another hospital. *I told that nurse I didn't feel right. My electrophysiologist excused the misreading of my EKG by the ED clinicians.
