The thing about the hemorrhoid surgery had more explanation in the real scene. Apparently a patient had hemorrhoids but also another deadly disease. The hemorrhoid surgery was not necessary for the guy as he had less time to live but he still went through with it to live the rest of his time comfortably. Hence, the comparison with the meeting with Leah's parents. As it's not necessary but does relieve them knowing they did it.
