To continue on the story of Old One Eye, it’s been reported that the monster has been seen from planet to planet. To which there’s two theories on why. The first one being it consumed some Crotalids, which are essentially massive crocodiles that are able to somehow travel through the warp, and gained their special ability. The other being the hive mind has noticed the fear old one eye brings, and so chooses to remake it similar to the swarm lord in how it can come back after being killed. Though I haven’t read much on his lore, so if there’s any corrections, feel free to tell me.

I think the original one eye is dead but the hivemind keeps evolving One Eyed Carnifexes for the psychological effect it has on the imperium. A rare instance of personality from the hivemind.

Perhaps the many old one eyes are due to the hive mind not actually being able to figure out what initiated its regenerative abilities, so it simply repeates the injuries the original suffered in order to activate the ability

My head canon is that old one eye purposely keeps it’s eye “damaged/missing” to intimidate the population and just uses sensors around it to continue seeing from its injury side
