Pretty sure this is not how a Meisterstück ballpoint should open. Most likely the glue holding the nose onto the body has failed and it needs to be sent in for service. You should be able to unscrew the cap by turning it counter-clockwise.

I have this exact same pen which recently started skipping. It's 20 years old, I keep it in my wallet/checkbook, only used to write checks. THANK YOU for this video, I will order a new ink cartridge and follow your guidance. 😊
