I'd like to comment on three of these topics:
New Coke: To rub salt into their (Coke's) wounds, Pepsi ran an ad where a life-long Coke drinker tasted his first Pepsi and stated: "Now I know why Coke did it!"
Russia selling Alaska: Considering the alternative, fighting an expensive war with Britain (Canada) and possibly losing Alaska anyway, selling it, even to this bargain basement price, was the better option.
Chernobyl: The disaster was actually made known for the outside world thanks to Swedish nucelar workers, who got readings about elevated radioactivity on the external sensors on the plant they were stationed at. After checking everything, and finding out that nothing was wrong with their own plant, they alerted the Radiation Protection Agency (Strålskyddsinsitutet, SSI) and after a check with meteorological data, they had enough facts to ask the Soviets: "Do you have a problem in Ukraine?"

The Titanic cost four hundred million to construct adjusted for inflation, and first class rooms cost five thousand dollars. That's a lot for a ship that didn't even complete one crossing.

Zero Hour is a very good series, and the episode on Chernobyl is particularly good. As it points out, THE SHIFT WORKERS were NOT to blame. It was a party member who insisted on running a test by pushing things beyond safety limits. The shift workers tried to stop him, and their jobs were threatened.

Actually New Coke wasn’t really an expensive mistake, it didn’t really hurt them as much financially. I would put the KKR buying Nabisco for billions of dollars, only to later sell the company to Kraft several years later. Blockbuster’s failed buyout of Netflix was mostly the biggest missed opportunity and financial mistake, but rather than the worst buyout like with AOL Time Warner. Also Daimler Chrysler was such a big financial mistake that it bankrupt Chrysler with unreliable vehicles!

I can understand in a way why you put Apollo 13 on this list but I think Challenger should have been on the list rather than Apollo 13. 7 people dying outranks, on the "severity of error" scale, 3 people getting home safe

The new Coke can’t really be considered a failure at all because when they went back to the original sales skyrocketed so much it made up for any loss and then some.

Without any discussion the most expensive mistake:
When the driver from Arch Duke Franz Ferdinand turned into the wrong street just to drive past Ferdinands assassin, resulting in WW1 and later down indirectly into WW2.

Embarrassing mistakes in train procurement have also been an issue in Australia in recent years. The New South Wales State government ordered a fleet of new trains that were found to be too wide to pass through tunnels, while the Queensland State Government ordered a fleet of new trains that had to be rebuilt after a disabled person successfully sued under the Anti-Discrimination Act because the train carriages weren't wheelchair-accessible.

Blockbuster Video should be on the list. Blockbuster could have bought Netflix for 40 million dollars and turned it down. Now Netflix is worth hundreds of billions of dollars and where is Blockbuster Video now.

“ The simple fact was really nobody liked the new coke”

Dude that’s not even remotely close to true. this video itself even stated new coke routinely beat classic Coke in taste tests and I’ve had it before; it was really good (you can still sample it at the coke plant in Atlanta). The actual reason it was such a failure is because they didn’t keep classic Coke on the market to give people an option; they just took the original off the shelves altogether and replaced it with new Coke. Had they made both options available then the new coke would probably still be around.

Probably a trillion $ mistake was Apple giving AT&T the exclusive rights to sell the I phone for its first two years. If Apple had let all of wireless carriers sell the I phone, their might not be Android phones today.

I'm really sick and tired of people telling the lie about Coke and new coke. People loved new coke, it tasted better than Coca-Cola did. The reason it failed is because instead of introducing New Coke and giving people the option to buy the classic or the new coke, they shoved it down people's throats and in this country if you don't allow us to choose we will reject what you give us. That is the only reason New Coke failed, because Coke didn't think to have both products available and let the public truly decide. Have they done it that way we'd all be drinking New Coke today because it truly did taste better
