In reality, the head engineer refused to sign off on the launch due to the risk imposed by frozen O rings. His boss overruled him and signed off on the launch. A sad, preventable loss of life in NASA history.

The Transocean Winner was the first semi submersible rig from the Norwegian sector to be scrapped. I was the second to the last person to leave the rig before the tugboat took it away. This incident happened on that voyage, in August 2016. The route was planned to be Norway-Malta-Turkey.
The salvage operation was expensive, but no repairs were made since the rig was already decommissioned. Smit Salvage managed to keep the rig afloat by using four large compressors pumping air through the tanks in the pontoons.
After a few months delay, the rig was transported as planned, but on the deck of the heavy load vessel MV Hawk.

I was in tech school in Denver Co, when the Challenger exploded, I had just come down from my dorm room to the tv room where everybody was fixed on the big screen tv, and just as I turned to look what was going on, that's when they showed the space shuttle exploding. In my lifetime I have lived through 3 of the worst disasters to affect America, Mount St. Helens in 1980, the Challenger explosion in 1986, and finally 9/11. Not to mention other disasters that have happened world wide, the Chernobyl disaster also in 1986, the Fukushima Daiichi nuclear disaster/earthquake in 2015, Hurricane Katrina, and the list goes on and on. This is just insane.

Mrs Richards: " I paid for a room with a view !"
Basil: (pointing to the lovely view) "That is Torquay, Madam. "
Mrs Richards: "It's not good enough!"
Basil: "May I ask what you were expecting to see out of a Torquay hotel bedroom window? Sydney Opera House, perhaps? the Hanging Gardens of Babylon? Herds of wildebeest sweeping majestically past?..."
Mrs Richards: "Don't be silly! I expect to be able to see the sea!"
Basil: "You can see the sea, it's over there between the land and the sky."
Mrs Richards: "I'm not satisfied. But I shall stay. But I expect a reduction."
Basil: "Why?! Because Krakatoa's not erupting at the moment?

Oh there were definite concerns not to launch due to the weather. What’s extra sad about that challenger disaster is that those astronauts likely all survived the initial explosion even possibly conscious but had no way to survive the fall due to the high cost and belief that the chances of failure, and thus need of a crew escape ability, was low.

I thought that transocean rig was being taken for scrap when it ran around.

The Costa Concordia disaster happened just because of a sail-by salute, too close...

The managers at NASA were told how dangerous the launch could be due to the cold....but they didn't listen. What is even more disturbing is how the bodies of the 7 on Challenger were collected. Literally put in garbage bags and then into the back of a damn truck. Hell of a way to treat them, humanity is gross.

Throw in the Hubble lens. Although I'm glad it's fixed!

I remember watching the shuttle explode while seeing it on tv at school.

The most expensive mistakes all in the US. You see what happens in a society where money is the lead thing.

Did anybody actually get fired for *any* of these disasters? The lack of accountability in today's society is outrageous, and ultimately fatal.

I’ve been traveling to visit Disney World on a yearly or bi-0yearly basis since at least 1993 and I have never heard of Splendid China.

9:10 They also don't believe in control burns. Which is a good 75-80% of their wild fire problems.

The narrator’s voice sounds so much like Chris Pine.

“No mind was paid to it” That’s wrong, they told managers that it would be dangerous to launch.

I remember the oil rig incident. Was on our island (isles of lewis) police had the village cordoned off but we pretended to have a delivery to 1 of the houses so we could get a closer look. The team world of everyone involved was fantastic in recovering it

I live in the Tampa Bay area and have never heard of Splendid China. I know there used to be a small amusement park in the Kissimmee area called "Circus Circus" many years ago. Then it changed to some other place I think was called "Boardwalk and Baseball". The main attraction for our family was the big wooden rollercoaster that would jolt every bone and joint on your body. These both closed before this Splendid China was even built.
I remember being at work in the kitchen of a nursing home when the Challenger exploded. Our boss told us to come to the activity room to see the TV coverage. Very sad day.
