Actually the pilot was clear for takeoff and Pan Am was simply instructed to follow KLM and takeoff, but since there was fog there was a misunderstanding and the two planes collided with each other

The true cause is
My guess might be from ATC error due to communication problems/fog, KLM was not insured because the ATC cannot see the planes.

Pan am had 61 survivors some people in the klm survived the impact but the seatbelt’s where jammed and the klm became a inferno while sliding on the runway
