So You Want to Be a FAMILY MEDICINE DOCTOR [Ep. 28]

So you want to be a family medicine doctor. You like the idea of primary care, working with patients of all age ranges, and like Vin Diesel, you don't have friends, you got family. Let's debunk the public perception myths, and give it to you straight. This is the reality of family medicine.

Family medicine is the center of primary care, the generalists of generalists. Unlike other specialties that are limited to a particular organ, disease, or age range, family physicians treat the broadest range of ailments and patients, from newborns to seniors, and everything between.

After medical school, family medicine residency is 3 years. It's categorical, meaning you match into one program and complete all three years there, including your intern year.

If you're the medical student who went through third year and loved everything non-surgical and didn't want to give any of it up, including not only organ systems but also age, then family medicine may be a good fit for you.

Currently premed at 30 but becoming a rural family medicine physician has been my dream for a long time. Feed the chickens and milk the goats then go be the best damn doctor I can to my little community (wherever I end up).

Big thanks for addressing the ego issue with med students and FM. I scored 550 on my COMLEX and people look down at me for loving FM and Peds, saying I'm wasting potential. It's nice to see you acknowledge the skill and knowledge it takes to do well as a Family physician

FM is crazy! Suture in AM, kick count at brunch, pull out foreign body in ear before lunch, check prostate after lunch, pap after, refill thyroid meds, prescribe fluoride late afternoon, and review colonoscopy results with grandma. LOVE IT!!!

I have no desire to be a doctor, but if I were to become one, I’d do family medicine. The flexibility and ability to know and do a bit of everything is something that’s very attractive and nothing to sneeze at.

When you're a premed: I WANNA BE A SURGEON
When you are a medical student doing clinicals: Family Medicine sounds much nicer.

I’ve been waiting for this video! I fell in love with family medicine after watching my dad practice in an underserved area and shadowing a family med physician who did bariatrics AND sickle cell disease management. AMAZING specialty and I can’t wait to go into it!

A few things I would add: subspecialties of Family Medicine vary in their scope and responsibilities.
1) OB alone without complex cases or C-section privileges can be done with simply getting enough cases in residencies. C-sections often require either a fellowship or 40 primary C-sections being performed at a full spectrum Family Medicine program that caters to OB.
2) ER can be done without a fellowship, again with enough rotations in residency, but that often is only at lower acuity ERs in smaller cities, towns and in rural areas. I'm not sure if a fellowship would allow for jobs in larger ERs.
3) Local high school Sports medicine can be done without a fellowship, but all Professional and Olympic doctors are fellowship trained.
4) To my knowledge, Hospice and Geriatrics usually don't require fellowships. In essence, Family Medicine is a rubber band. The specialty is flexible as Family Docs must be capable to see every age and assist with many procedures in lower density areas like towns and rural areas. That said, if you find yourself in urban USA, you can easily focus on one aspect of medicine. In effect, few family doctors are true generalists anymore. OB-Peds, Adult, ER, Sports, Hospice, etc, all require specific skills and no doctor can manage them all BUT family doctors scope is broader (and as a necessary consequence not deeper) than most specialists.

In terms of respect and prestige, its depends. As a PGY-3 in Family Medicine, I can honestly say that a good family doctor is respected. Patients do not seem to care about MD, DO or even PA, ARNP etc as long as they get good care. Obviously, prestige seems higher in surgical specialties (as does money), but at the end of the day that happiest docs love what they do and surround themselves with individuals who also value it. Working in underserved areas is a great way to gain that intrinsic value and communal engagement in your work. That said, patients may be more demanding of credentials or question you more if they are upper middle class (though I haven't seen that very much personally).

For those considering family medicine, ask yourself if you like everything. If you want the option to do OB or ER in addition to other things. If you like to be the base of the healthcare system. We are often unsung heroes who don't receive the praise of MFM, Ortho or NICU, but who are truly the most valuable to healthcare. That said, if you want to do IM fellowships or anything surgical other than C-sections, family medicine is not for you. Peace

This is excellent. I've had to have many specialists, but now tired of going to so many docs, my local doc (known him all my life) for family medicine, IS THE BEST. I'm in a rural area, and he tackles everything of mine! They were in a private practice, but a larger system bought them out, bc they are older (70s! but still awesome & there's a 2nd one - same practice - in my small town with him & then there's the small hospital with a clinic & excellent ER that is essential for our location.) My Dr is an MD, his partner Dr is a DO. My Dr is actually the one who has helped me be better, even after all the health issues in the past & genetic, he listens to me & just cares. He's been wealthy bc back in the day, they made more (late 70s & 80s, they were able to be more customized & see more ppl bc they didn't have to deal with computers yet lol) plus he has been a great Dr. You were so accurate with this video! They definitely have to be into helping & listening to all demographics of ppl bc they get EVERYTHING. ❤️

I KNOW ABOUT THE FEELING OF LOOKING DOWN ON FAMILY PHYSICIANS. WAS ASKED RECENTLY WHY I WOULD CHOOSE TO DO FAMILY MEDICINE WHEN I AM CAPABLE OF ACHIEVING MUCH MORE. TRUTH IS, WHAT'S NOT TO LOVE? YOU GET TO DO EVERYTHING WITHOUT RESTRICTIONS OF AGE, SEX, ORGAN SYSTEM, OR TYPE OF DISEASE. IT'S AN AMAZING SPECIALTY AND I AM REALLY EXCITED ABOUT MY DECISION TO GO INTO THIS FIELD!
